摘要
目的分析伴轻中度耳鸣的听力正常青年人可接受噪声级(acceptable noise level,ANL)的特点,探讨耳鸣对ANL测试的影响。方法采用汉语普通话测试材料对26例伴轻中度耳鸣的听力正常青年人(耳鸣组)和44例听力正常青年人(正常组)进行ANL测试,分别获得最舒适响度级(most comfortable loudness,MCL)和最大背景噪声级(background noise level,BNL),根据公式:ANL=MCL-BNL,计算出ANL值,比较两组的MCL、BNL、ANL值,并对耳鸣组耳鸣残疾量表(tinnitus handicap inventory,THI)得分与ANL值进行相关性分析。结果耳鸣组和正常组的MCL值分别为41.98±8.90、39.51±7.46dB HL,BNL值分别为40.92±11.71、36.97±8.82dB HL,ANL值分别为0.04±6.89、2.43±5.84dB,两组的MCL、BNL、ANL值差异均无统计学意义(P>0.05);两组的MCL与ANL均无相关关系(P>0.05),BNL与ANL均呈明显负相关(r值分别为-0.67和-0.52,P<0.05);耳鸣组的THI值与ANL值无明显相关性(r=0.057,P>0.05)。结论伴轻中度耳鸣的听力正常青年人的耳鸣症状对ANL测试无影响,且THI值与ANL值无明显相关性。
Objective To study the characteristics of acceptable noise level（ANL）in young adults with mild to moderate tinnitus and to investigate the effect of tinnitus on ANL test.Methods The Mandarin proficiency test material was used in this study.ANL tests were performed on 26 young normal hearing adults with mild to moderate tinnitus（tinnitus group）and 44 normal hearing adults（normal group）.For each case,the most comfortable loudness（MCL）and the background noise level（BNL）were obtained.ANL is MCL minus BNL.The MCL,BNL and ANL values of the two groups were compared.Results The MCL results of young adults with tinnitus were 41.98±8.90 while normal hearing adults were 39.51±7.46 dB HL.The BNL were 40.92±11.71 and 36.97±8.82 dB HL.The ANL were 0.04±6.89 and 2.43±5.84 dB.There was no significant difference in MCL,BNL and ANL between two groups（P〉0.05）.There was no correlation between MCL and ANL in two groups（P〉0.05）.But there was a significant negative correlation between BNL and ANL in two groups（r=-0.67 and-0.52,P〈0.05）.There was no significant correlation between ANL and THI in young normal hearing adults with tinnitus（r=0.057,P〉0.05）.Conclusion Mild to moderate tinnitus had no effect on ANL test,and the THI value had no significant correlation with ANL value.
