带线铆钉联合空心螺钉治疗后交叉韧带胫骨止点撕脱骨折18例 被引量:4

摘要 目的:探讨带线铆钉结合空心螺钉治疗后交叉韧带胫骨止点撕脱骨折的手术方法及临床疗效。方法:对18例后交叉韧带胫骨止点撕脱骨折采用取膝关节后方切口,直视下复位骨折,使用空心螺钉固定骨折块、带线铆钉重建后交叉韧带。术后使用Lysholm评分、膝关节活动度、Lachman试验及后抽屉试验评价膝关节功能恢复情况。结果:18例均获得随访,随访8~24个月,平均(14.2±3.2)个月。术后复查X线片显示所有患者骨折复位满意,骨折均顺利愈合,平均愈合时间2.5个月。Lysholm评分术前与术后分别为(52.56±7.82)分、(90.32±3.47)分,差异有统计学意义(P<0.05);膝关节活动度术前与术后分别为(41.73±7.92)°及(125.52±3.51)°,差异有统计学意义(P<0.05);术后Lachman试验及后抽屉试验均为阴性。结论:带线铆钉结合空心螺钉治疗后交叉韧带胫骨止点撕脱骨折的手术创伤小,固定牢固可靠,利于早期康复锻炼及功能恢复,值得临床推广。
