摘要
目的:贫血与慢性心衰患者远期不良事件密切相关,但贫血对心衰早期的影响尚未明确,探讨贫血对首诊急性心力衰竭患者1年死亡率的影响。方法:纳入2012年1月-2016年9月在本院首次住院的500例急性心力衰竭老年患者,根据是否贫血分为贫血组(310例)与非贫血组(190例)。对所有患者进行为期1年的随访,通过Cox回归分析贫血与HF患者不良预后的关系。结果:贫血组住院时间明显大于非贫血组,差异有统计学意义(P<0.01)。贫血组30 d死亡率8.7%高于非贫血组3.7%,差异有统计学意义(X^2=4.694,P=0.030)。贫血组1年死亡发生率28.1%高于非贫血组18.4%,差异有统计学意义(X^2=5.939,P=0.016)。通过Cox回归分析,贫血是急性HF患者1年死亡率的风险因子[HR=1.778,95%CI(1.183,2.601),P<0.001]。结论:贫血是首次住院急性心力衰竭患者远期不良事件经济有效观察指标。
Objective:To discuss the effect of anemia on the mortality rate of patients with acute HF in one year after initial diagnosis since anemia is closely related to long-term adverse events of patients with chronic HF,but the influence of anemia on the early phase of HF is still uncertain.Method:A total of 500 elderly patients with acute HF in our hospital from January 2012 to September 2016 were selected in this research,and they were divided into anemia group and non-anemia group according to whether they suffered from anemia.A follow-up visit was paid to all patients for one year,and the relation between anemia and poor prognosis of HF patients was analyzed through Cox regression.Result:There was no statistical difference between anemia group and non-anemia group in hospital stays(P〈0.05).The mortality rate in 30 days of anemia group(8.7%) was higher than that of nonanemia group(3.7%),the difference was statistically significant( χ2=4.694,P=0.030).The mortality rate in one year of anemia group(28.1%)was higher than that of non-anemia group(18.4%),the difference was statistically significant(χ2=5.939,P=0.016).According to Cox regression analysis,anemia was the risk factor of mortality rate of patients in one year[HR=1.778,95%CI(1.183,2.601),P〈0.001].Conclusion:Anemia is an economic and effective observation indicator for long-term adverse events of acute HF patients hospitalized for the first time.
