摘要
目的调查分析医院中心药房患者退药原因,总结减少退药的措施。方法分析医院中心药房退药患者600例的退药资料,对退药原因进行分类统计并计算构成比。结果退药主要原因为患者出院或转科、药物不良反应、医嘱调整、医嘱输入错误、不合理用药、患者死亡和取消检查等。结论医院应采取措施规范退药程序,加强医患沟通,提高合理用药水平,降低退药率,以提高医疗资源利用率。
Objective To investigate the drug withdrawal analysis and improvement measures of center pharmacy in hospital of 2017. Methods 600 cases of drug withdrawal information of 600 were analysised in the center pharmacy. Classify the reasons for drug withdrawal and calculate the composition ratio. Results The main reason for drug withdrawal was patient discharge or transfer,adverse drug reactions,medical adjustment,medical input error,irrational drug use,the patient died and cancelled the inspection. Conclusion Hospitals should take measures to standardize drug withdrawal procedures,strengthen communication between doctors and patients,increase the level of rational drug use,and reduce the rate of drug withdrawal,so as to increase the utilization rate of medical resources.
