11249株临床分离革兰氏阴性杆菌耐药性分析 被引量:4

摘要 目的了解宁夏人民医院临床分离革兰氏阴性杆菌的分布和耐药性,指导临床合理使用抗菌药物。方法应用VITEK 2 Compact全自动微生物鉴定系统和生物梅里埃API鉴定系统,结合K-B法药敏试验对临床分离革兰氏阴性杆菌进行鉴定和药敏试验,按CLSI 2015年版标准判读结果,用WHONET 5.6软件进行统计分析。结果收集2012年1月-2016年12月临床分离的革兰氏阴性杆菌共11 249株,分离率最高的为大肠埃希菌(3 477株,30.9%);呼吸道标本以外的其他标本均以大肠埃希菌为主。除了阴沟肠杆菌对厄他培南的耐药率为11.3%,其他肠杆菌科细菌对碳青霉烯类抗生素的耐药率为0.2%~1.3%。铜绿假单胞菌除替卡西林/克拉维酸和氨曲南以外,对其他抗菌药物耐药率均低于20%。鲍曼不动杆菌除对阿米卡星的和米诺环素以外,其他抗菌药物耐药率均已超过了50%。结论宁夏人民医院临床分离革兰氏阴性杆菌以大肠埃希菌最常见,鲍曼不动杆菌等部分细菌耐药严重,应加强耐药监测,为临床选择抗菌药物提供依据。 Objective To investigate the distribution and drug resistance of gram-negative bacilli isolated from the clinical stuff in Ningxia People's Hospital,so as to guide the rational use of antibiotics in clinical treatment. Methods VITEK 2 Compact automated microbial identification system,bio Merieux API identification system and K-B method were applied for the identification of the clinical isolates of gram negative bacteria and analysis of the drug susceptibility of them. Results were interpreted according to the judging standard established by CLSI,2015 version. Statistical analysis was performed by WHONET5. 6 software. Results A total of11,249 strains of gram-negative bacteria were isolated in the Ningxia People's Hospital during January 2012 and December 2016.Escherichia coli has the highest isolation rate(3477 strains,30. 9%). Escherichia coli is the most common isolate of specimens except respiratory tract specimens. Besides the resistance rate of Enterobacter cloacae resistant to ertapenem was 11. 3%,the resistance rate of the rest Enterobacteriaceae to carbapenems were in 0. 2% ~ 1. 3%. The resistance rate of the Pseudomonas aeruginosa to antibiotics was lower than 20%,expect for picacillin/clavulanic acid and aztreonam. All the resistance rate of Acinetobacter baumannii to other antibiotics had exceeded 50% except for Amikacin and minocycline. Conclusion Escherichia coli is the most common clinical isolates of gram-negative bacilli in the Ningxia People's Hospital. Drug resistance of certain bacteria such as Acinetobacter baumannii is serious.The monitoring of drug resistance should be strengthened to provide the basis for clinical selection of antibiotics.
