减压融合内固定术联合椎间融合器治疗退变性腰椎管狭窄的效果分析 被引量:5

摘要 目的探讨减压融合内固定术联合椎间融合器治疗退变性腰椎管狭窄的效果并作分析。方法将自2013年11月—2015年11月期间收治100例退变性腰椎管狭窄患者纳入研究并以就诊单、双月分为两组。对照组仅接受椎间融合器治疗,研究组则应用减压融合内固定术联合椎间融合器治疗,并观察比较两组的治疗前、治疗3个月后、治疗2年后的腰痛VAS、腿痛VAS、脊椎ODI评分、椎间隙高度、Cobb角。结果研究组治疗3个月后腿痛VAS评分、腿痛VAS评分、脊椎ODI评分分别为(2.50±1.50)分、(2.30±1.01)分、(4.05±2.15)分及治疗2年后腿痛VAS评分、腿痛VAS评分、脊椎ODI评分分别为(2.70±1.50)分、(2.30±1.40)分、(5.20±2.05)分,均低于对照组(P<0.05)。研究组治疗3个月后椎间隙高度(7.10±1.55)mm、治疗2年后椎间隙高度(7.00±0.90)mm均低于对照组(P<0.05)。研究组治疗3个月后Cobb角(10.25±2.20)°、治疗2年后Cobb角(10.35±2.35)°均与对照组差异无统计学意义(P>0.05)。结论退变性腰椎管狭窄患者应用减压融合内固定术联合椎间融合器治疗效果显著。 Objective This paper tries to investigate the effect of decompression and fusion internal fixation combined with interbody fusion for the treatment of degenerative lumbar spinal canal stenosis. Methods A total of 100 patients with degenerative lumbar spinal stenosis from November 2013 to November 2015 were included in the study and were divided into 2 groups by single bimonthly. The control group received interbody fusion treatment, the study group adopted decompression fusion and internal fixation combined with interbody fusion treatment, and the treatment of two groups were compared before and after 3 months of treatment, after 2 years of treatment of low back pain, leg pain,spinal VAS VAS ODI score, intervertebral height, Cobb angle were compared. Results In the study group for 3 months and back leg pain VAS score, VAS score, ODI score of spinal pain [(2.50 ±1.50)points,(2.30±1.01)points,(4.05 ±2.15)points] and after 2 years of treatment of leg pain VAS score, pain score, vertebral VAS ODI score [(2.70 ±1.50)points,(2.30±1.40)points,(5.20±2.05)points] were lower than the control group(P〈0.05). After 3 months of treatment, the height of the intervertebral space(7.10 ±1.55)mm and the height of the intervertebral space(7.00 ±0.90)mm after 2 years of treatment were lower than those in the control group(P〈0.05). After 3 months of treatment in the study group, the Cobb angle(10.25±2.20)° and the Cobb angle(10.35±2.35)° after 2 years of treatment were not significantly different from those in the control group(P〉0.05). Conclusion The effect of decompression and fusion internal fixation combined with interbody fusion is significant in the patients with degenerative lumbar spinal canal stenosis.
