摘要
目的观察神经肌肉刺激治疗仪治疗盆腔淤血综合征的临床效果。方法选择2014年1月至2015年6月收治的盆腔淤血综合征患者100例,所有患者均经临床症状、阴道彩色多普勒超声等检查确诊为盆腔淤血综合征。将所有患者随机分为2组,每组50例。对照组患者采用胸膝卧位来缓解盆腔淤血,2次/d,一般每天中午和晚上坚持做15 min,随后采取侧俯卧位休息。试验组在对照组治疗的基础上采用神经肌肉刺激治疗仪促进血液循环治疗,观察患者临床症状、B超检查、妇科检查等评价2组患者的治疗效果。结果试验组总有效率为96.0%,高于对照组的80.0%,差异有统计学意义(P<0.05)。治疗前试验组和对照组患者盆腔静脉血流速度和盆腔静脉内径比较差异无统计学意义(P>0.05)。治疗后2组盆腔静脉血流速度均明显加快,与治疗前比较差异有统计学意义(P<0.05);治疗后2组患者盆腔静脉内径均明显缩小,但与治疗前比较差异均无统计学意义(P>0.05);治疗后2组盆腔静脉血流速度差异有统计学意义(P<0.05),治疗后2组盆腔静脉内径比较差异无统计学意义(P>0.05)。治疗前2组症状评分和疼痛程度比较差异均无统计学意义(P>0.05),治疗后2组患者症状评分均明显降低,且试验组低于对照组,差异均有统计学意义(P<0.05)。治疗后试验组无痛率高于对照组,差异有统计学意义(P<0.05)。结论神经肌肉刺激治疗仪PHENIX U4治疗盆腔淤血综合征疗效显著,可显著改善患者盆腔静脉的血液循环,明显减轻盆腔静脉淤血程度,进一步缓解盆腔淤血综合征引起的慢性盆腔疼痛症状,值得临床广泛推广应用和研究。
Objective To observe the clinical effects of neuromuscular stimulation therapeutic apparatus on pelvic congestion syndrome. Methods One hundred patients with pelvic congestion syndrome who were admitted and treated in our hospital from January 2014 to June 2015 were enrolled in the study. All the patients were diagnosed as pelvic congestion syndrome by clinical symptoms,vaginal color Doppler ultrasonography. These patients were randomly divided into control group and experimental group,with 50 patients in each group. The patients in control group were treated by chest and knee position to relieve the pelvic congestion,15 min once,twice a day,usually at noon and at night,then taking the lateral prone position to rest,however,the patients in experimental group,on the basis of control group,were treated by neuromuscular stimulator to promote blood circulation. After treatment,the clinical symptoms,B ultrasonic examination results and gynecological examination results were observed to evaluate and compare the therapeutic effects between two groups. Results The total effective rate in experimental group was 96%,which was significantly higher than that( 80%) in control group( P〈 0. 05).Before treatment there were no significant differences in pelvic venous flow rate and pelvic vein diameter between the two groups( P〉 0. 05). After treatment the pelvic venous blood flow velocity was significantly accelerated in both groups,as compared with that before treatment( P〈 0. 05),moreover,the pelvic vein diameter was decreased in both groups after treatment,but there was no significant difference( P〉 0. 05). After treatment there was significant difference in pelvic vein blood flow velocity between two groups( P〈 0. 05),however,there was no significant difference in pelvic vein diameter between two groups( P〉 0. 05). Before treatment there were no significant differences in symptom scores and pain degree between two groups( P〉 0. 05),however,after treatment,symptom scoress were significantly decreased in both groups,moreover,which in experimental group were significantly lower than those in control group( P〈 0. 05). In addition the painfree rate after treatment in experimental group was significantly higher than those in control group( P〈 0. 05). Conclusion The neuromuscular stimulation therapeutic instrument( PHENIX U4) has obvious therapeutic effects on pelvic congestion syndrome,which can significantly improve blood circulation of pelvic vein,relieve the pelvic venous congestion degree to alleviate the chronic pelvic pain symptoms caused by pelvic congestion syndrome,therefor,which is worthy of clinical application and promotion.
