摘要
目的探讨枸橼酸咖啡因治疗早产儿原发性呼吸暂停的疗效并观察不良反应发生情况。方法选取2016年3月~2017年11月期间于我院新生儿科接受治疗的120例原发性呼吸暂停早产儿为研究对象,随机分成对照组及观察组。对照组56例,给予氨茶碱治疗;观察组64例,给予枸橼酸咖啡因治疗。连续治疗7d后,比较和分析两组患儿的临床疗效,观察不良反应发生情况及两种用药治疗方法对患儿早期肺功能的影响。结果连续治疗7d后,观察组总有效率达93.75%,明显高于对照组(71.43%),差异有统计学意义(P<0.05);观察组潮气量、每分通气量、达峰时间比、达峰容比及75%、50%、25%潮气量时呼气流速均明显高于对照组,差异均有统计学意义(P<0.05);在治疗过程中,观察组患儿不良反应发生率达7.81%,明显低于对照组(17.86%),差异有统计学意义(P<0.05)。结论枸橼酸咖啡因在治疗早产儿原发性呼吸暂停方面的疗效要优于氨茶碱,且安全性较高,能够明显改善患儿早期肺功能,可以作为治疗早产儿呼吸暂停的优选药物。
Objective To observe the effect and adverse reactions of caffeine citrate in the treatment of primary apnea in premature infants. Methods From March 2016 to November 2017, 120 cases of primary apnea preterm infants who underwent neonatal treatment in our hospital were selected as the research object,and were divided into control group and observation group according to the treatment method. The control group(n=56) received aminophylline treatment, the observation group(n=64) was received caffeine citrate treatment. After continuous treatment for 7 days, the clinical effects of two groups were compared and analyzed, the incidence of adverse reactions and the effects of the two methods of treatment on early pulmonary function were observed. Results After continuous treatment for 7 days, the total effective rate in the observation group reached 93.75%, which was significantly higher than that in the control group(71.43%), P 〈0.05. The tidal volume, minute ventilation, peak time ratio, peak volume ratio, TEF75, TEF50 and TEF25 in the observation group were significantly higher than those in the control group (P 〈0.05). The incidence of adverse reactions in the observation group was 7.81%, which was significantly lower than that in the control group(17.86%), the difference was statistically significant(P 〈0.05). Conclusion Caffeine citrate is better than aminophylline in the treatment of premature infants with primary apnea, and it has high safety, can significantly improve the early pulmonary function in children. It can be used as the best drug in the treatment of premature infants with apnea.
