Dynamic Contrast-Enhanced Magnetic Resonance Imaging in Denervated Skeletal Muscle: Experimental Study in Rabbits

摘要 目的运用动态增强(DCE)MRI评价兔失神经骨骼肌的特点。方法 12只兔随机平均分成实验组和对照组,分别与术前、术后第1天、3天、5天、1周、2周、3周、4周、6周、8周、10周、12周行兔下肢磁共振扫描。在实验组,观察左侧腓肠肌T2WI信号变化。组内DCE-MRI参数容量转移常数(Ktrans)、反流速率常数(Kep)、血浆容积分数(Vp)运用重复测量方差分析。组间参数运用单因素方差分析。结果在实验组,左侧腓肠肌T2WI信号增高。Ktrans,Vp逐渐升高至术后第4周并且维持高位至第12周。和对照组比较,Ktrans,Vp明显增高(P〈0.05)。Kep在组内和组间均无统计学意义(P〉0.05)。结论 Ktrans和Vp在骨骼肌失神经的变化明显早于肌电图(EMG)和MR常规序列,DCE-MRI可以用来早期诊断失神经骨骼肌。 Objective To investigate the value of dynamic contrast-enhanced( DCE) magnetic resonance( MR) imaging for evaluating denervated skeletal muscle in nerve injury models in rabbits. Methods Twelve male rabbits were randomly divided into an experimental group and a control group. MR imaging of rabbit lower legs was performed before nerve surgery and 1 day,3 days,5 days,1 weeks,2 weeks,3 weeks,4 weeks,6 weeks,8 weeks,10 weeks,and 12 weeks after surgery. Signal intensity changes were seen in the left gastrocnemius muscle on the T2-weighted MR images. DCE MR imaging derived parameters(Ktrans,Kep,and Vp) of specific acquisition points within groups were compared by using repeated-measures analysis of variance. Comparisons across the two groups at each acquisition point were performed by using one-way analysis of variance. Results In the experimental group,T2-weighted images showed increased signal intensity in the left gastrocnemius muscle. Ktrans,Vp values increased gradually until 4 weeks after surgery and remained high throughout the remainder of the study period. There are significant increases in both Ktrans and Vp values compared with those in the control group after surgery( P 〈 0. 05). Kep values show no significant difference between the irreversible neurotmesis group and the control group. Conclusion Increased Ktrans and Vp values is an early phenomenon markedly preceding not only EMG abnormalities but also signal intensity changes in denervated skeletal muscle after nerve transection.
