摘要
目的回顾性分析67例亚急性甲状腺炎患者的临床与核医学结果,评价核医学检查在亚急性甲状腺炎诊断和鉴别诊断中的应用价值。方法本组67例肉芽肿性(GST)和无痛性(PST)亚急性甲状腺炎患者均进行甲状腺激素测定、131I摄取率(RAIU)和放射性核素显像检查,部分患者行ESR及TGAb、TPOAb测定。结果所有病例3h和24h RAIU均显著降低;67例患者中66例出现血清学异常,1例在正常范围;58例患者核素显像出现不同程度的放射性减低,但GST组和PST组表现有所不同,且两组的TPOAb(P=0.004)、ESR(P<0.001)和RAIU<2%,结果(P<0.001)存在差异,具有统计学意义(P<0.05)。结论结合临床血清学免疫分析、RAIU检测和核素显像,不但有利于亚急性甲状腺炎的诊断,在GST和PST鉴别诊断中也具有一定的临床价值。
Objective To analyze the clinical and nuclear medical results of 67 cases of subacute thyroiditis retrospectively and to investigate the clinical value of nuclear medical examination in the diagnosis and differential diagnosis of subacute thyroiditis. Methods 67 patients with subacute thyroiditis, consisting of 50 patients with granulomatous subacute thyroiditis （GST） and 17 with painless subacute thyroiditis （PST）, all underwent the same thyroid hormone test, 131 I-intaking rate experiment （RAIU） and thyroid radionuclide imaging examination. Some patients were subjected to the test of ESR, TGAb, and TPOAb. Results RAIU at 3 rd and 24th hour was significantly decreased in all patients; serological abnormality, including T3, T4, TGAb, TPOAb and ESR, was shown in 66 patients; obvious reduction of radionuclide uptaking was observed in 58 patients. There were significant differences in TPOAb, ESR and RAIU〈2% between GST and PST groups （ P 〈0.05）. Conclusion Nuclear medcine, a combination of radioimmunology, 131 I-intaking rate experiment and thyroid imaging examination, can contribute to dignosis of subacute thyroiditis, as well as differential diagnosis of GST and PST.
