摘要
目的探讨快速康复外科理念(FTS)在老年胸腰椎压缩性骨折围手术期中的应用效果。方法选择东省立第三医院骨科于2013年10月至2017年11月期间收治的130例老年胸腰椎压缩性骨折患者为研究对象,根据随机数表法分为对照组和观察组,每组65例,所有患者均接受经皮椎体成形术治疗。对照组患者采用传统护理措施,观察组患者则应用FTS理念作为指导进行护理,比较两组患者术后疼痛程度[采用视觉模拟评分法(VAS)评价]、下床活动时间、住院时间、并发症发生率及满意度。结果观察组患者术后VAS评分及下床活动、住院时间分别为(2.3±0.8)分、(28.6±12.3)h、(5.7±2.1)d,并发症发生率为9.23%,均少于对照组的(3.8±1.9)分、(48.5±19.6)h、(7.1±3.7)d和18.46%,差异均有统计学意义(P<0.05);观察组患者的满意度为92.31%,明显高于对照组的81.54%,差异均有统计学意义(P<0.05)。结论对于老年胸腰椎骨折患者,在围手术期应用FTS理念作为指导,可以减轻患者的术后疼痛程度,减少并发症的发生率,促进术后康复,缩短住院时间,提高患者的满意度。
Objective To investigate the effect of fast track surgery（FTS） in the perioperative period of thoracolumbar compression fracture in elderly patients. Methods A total of 130 elderly patients with thoracolumbar compression fracture in the Department of Orthopedics, Third Hospital of Shandong Province from October 2013 to November 2017 were selected and divided into observation group and control group according to random number table, with 65 cases in each group. Based on percutaneous vertebroplasty treatment, the control group used the traditional nursing measures, and the observation group applied FTS concept as a guide. Postoperative pain by using Visual Analog Scale（VAS） assessment, ambulation time, hospitalization time, complication rate and satisfaction were compared between the two groups.Results Postoperative VAS score, ambulation, hospitalization time, and complication rate in the observation group were（2.3±0.8）,（28.6±12.3） h,（5.7 ±2.1） d, 9.23%, versus（3.8±1.9）,（48.5±19.6） h,（7.1±3.7） d, 18.46% in the control group（P〈0.05）. The satisfaction rate of the observation group was 92.31%, significantly higher than 81.54% in the control group（P〈0.05）. Conclusion For elderly patients with thoracolumbar fractures, perioperative FTS guidance can relieve postoperative pain, reduce complications, promote postoperative rehabilitation, shorten hospitalization time, and improve satisfaction.
