摘要
为了探讨复合术式(选择性痔上黏膜环切吻合术+内痔柱状缝扎术+皮赘弧形切除整形术)治疗重度环状混合痔的临床疗效,将2015年2月至2016年8月在乌鲁木齐市中医医院手术治疗的120例环状混合痔患者,随机分为治疗组和对照组,每组各60例。治疗组采用复合术式治疗,对照组采用外剥内扎术治疗。对2组的临床疗效进行评价,观察2组患者术后疼痛程度、创面水肿、瘙痒、潮湿及愈合天数的变化。结果显示,在术后第3、7天,2组患者疼痛症状评分比较,治疗组明显低于对照组,P<0.05,说明治疗组在缓解疼痛方面明显优于对照组;术后第1、3、7天水肿情况治疗组明显优于对照组,P<0.05;2组患者术后瘙痒、潮湿及愈合天数方面差异亦有统计学意义(P<0.05),但2组患者的临床总有效率均为100%(P>0.05)。结果表明,复合术式与传统术式均能有效治疗环状混合痔,但复合术式较传统术式更能提高手术疗效,减少术后疼痛及术后水肿等并发症的发生,更有利于促进病情康复。
This study was to investigate the clinical effect of the compound procedure （tissue-selecting therapy＋cylindrical suture-ligation for internal hemorrhoids-t-skin-tag arc-like resection ＆ plasty for se- vere of circumferential mixed hemorrhoids）, randomly divided 120 cases of this hemorrhoids who admitted and received surgery in author＇s hospital from Feb. 2015 to August 2016 yr. into treatment group and con- trol one（60 cases for each） , which the former＇s patients received above compound procedure, meanwhile the latter still adopted external dissection ~ internal ligation procedure, then evaluated and compared both group＇s clinical effect, and observed their postoperative pain degree, wound-surface edema, titillation and moisture,as well as healing time（days）.As results,on the 3rd,7th day after surgery in the score on pain theformer was significantly lower than the latter（ P 〈0.05） which showed pain relieved in the former was su- perior to in the latter,on the lst,3rd and 7th day after surgery in edema the former was superior to the lat- ter （ P 〈0.05） ;in postoperative titillation, moisture and healing time（days） there was statistical difference （ P 〈0.05）,but both group＇s total effective rate was all 100%（ P 〈0.05）.Results show that above-men- tioned procedures can all effectively treat circumferential mixed hemorrhoids,hut compound procedure can more uplift effect,reduce postoperative pain and edema, etc complication,thus more helpful to promote re- covery than routine procedure.
