两种运动模式器械预备根管疗效比较的系统评价 被引量:1

Clinical effect of rotary versus reciprocating instrument on root canal preparation: a systematic review

摘要 目的:系统评价使用往复运动模式系统Wave One和连续旋转运动模式系统Pro Taper预备根管的疗效。方法:计算机检索Pub Med、EMBASE、The Cochrane Library、CNKI、VIP和Wan Fang Data数据库,按照纳入、排除标准,搜集使用往复运动系统Wave One和连续旋转运动系统Pro Taper预备根管的相关临床随机对照试验(RCT),收集日期截止至2017年11月。根据Cochrane系统评价手册,对纳入研究进行偏倚风险评估,采用Rev Man 5.3软件进行Meta分析。结果:共纳入7个RCT,往复运动器械Wave One组(W组)共351例,连续旋转运动器械Pro Taper组(P组)共349例。Meta分析结果显示,两种运动模式系统根管预备术后疼痛发生率比较,差异无统计学意义(RR合并=0.97,95%CI=0.74~1.29),。两种系统轻度疼痛、中度疼痛和重度疼痛的发生率比较,差异无统计学意义(RR合并轻度疼痛=0.86,95%CI轻度疼痛=0.55~1.35;RR合并中度疼痛=1.23,95%CI中度疼痛=0.59~1.35;RR合并重度疼痛=0.73,95%CI重度疼痛=0.26~2.05)。Wave One预备根管所需的时间少于Pro Taper,差异有统计学意义(MD合并=-109.77,95%CI=-129.03^-90.52,P<0.001)。结论:使用往复运动模式器械Wave One和连续旋转运动器械Pro Taper预备根管的术后疼痛发生率接近,临床效果相似,但是往复运动器械Wave One预备根管的时间少于连续旋转运动器械Pro Taper。 Objective:To investigate the clinical effect of the reciprocating system Wave One and the rotary system Pro Taper in root canal preparation.Methods:Pub Med,EMBASE,The Cochrane Library,CNKI,VIP,and Wanfang Data were searched for randomized controlled trials(RCTs)on the application of the reciprocating system Wave One and the rotary system Pro Taper in root canal preparation published up to November 2017.Rev Man 5.3 was used to perform the meta-analysis according to the method of Cochrane systematic review.Results:A total of 7RCTs were included,with 351 patients in reciprocating system Wave One group(W group)and 349 patients in rotary system Pro Taper group(P group).The meta-analysis showed that the overall incidence rate of postoperative pain was 45.71%,and Wave One and Pro Taper had a similar incidence rate of pain after root canal preparation(risk ratio[RR]=0.97,95%confidence interval[CI]:0.74~1.29).The two groups had similar incidence rates of mild(RR=0.86,95%CI:0.55~1.35),moderate(RR=1.23,95%CI:0.59~1.35),and severe pain(RR=0.73,95%CI:0.26~2.05).Wave One had a significantly shorter time spent on root canal preparation than Pro Taper(MD=-109.77,95%CI:-129.03 to-90.52).Conclusion:The reciprocating system Wave One and the rotary system Pro Taper for root canal preparation have a similar incidence rate of postoperative pain and a similar clinical effect,but Wave One has a significantly shorter time spent on root canal preparation than ProTaper.
