摘要
目的探讨合理的临床路径在痔疮患者痔上黏膜环形切除钉合术(PPH)围术期管理的应用效果。方法选择2014年1月~2016年12月在中国中医科学院广安门医院南区肛肠科行痔疮PPH手术的患者200例作为研究对象,按照随机数字表法分为研究组与对照组,每组各100例。研究组给予临床路径管理,对照组给予常规诊治。对两组安全性、临床疗效、平均住院天数、平均住院费用及患者满意度进行比较。结果两组安全性、临床疗效比较,差异无统计学意义(P>0.05)。研究组平均住院天数、平均住院费用低于对照组,满意度高于对照组,差异均有高度统计学意义(P<0.01)。结论 临床路径作为一种科学的医疗护理模式,在PPH手术围术期管理中切实可行,而且提高了诊疗水平,具有一定的科学价值和积极的社会意义,值得推广。
Objective To discuss the application of rational clinical pathway in the perioperative management of procedure for prolapse and hemorrhoids(PPH) in patients with hemorrhoids. Methods From January 2014 to December2016, 200 patients underwent PPH therapy in the Department of Proctology, South District Guang'anmen Hospital,China Academy of Chinese Medical Sciences were divided into the study group and the control group according to the random number table, with 100 cases in each group. The study group applied the clinical pathway management and the control group applied the routine diagnosis and treatment. The safety, clinical efficacy, average length of hospital stay,average hospitalization cost and patient satisfaction were compared between the two groups. Results There were no significant differences in the safety and clinical efficacy between the two group(P〉0.05).The average hospital stay and the average hospitalization cost in the study group were lower than those of control group, the satisfaction rate in the study group was higher than that of control group, with highly statistically significant differences(P〈0.01). Conclusion Clinical pathway as a scientific medical care model applied in PPH of the perioperative management is practical and has certain scientific value and positive social significance. It can improve the level of diagnosis and treatment and is worthy of promotion.
