摘要
目的探讨新生儿性别、胎龄、出生体重与先天性甲状腺功能减低症(CH)筛查效率和发病率的关系,为预防新生儿CH的发生和优生优育咨询服务提供参考依据。方法对2015年1月至2017年12月广州市新生儿疾病筛查中心CH的筛查结果进行回顾性分析。探讨不同性别、胎龄和出生体重间新生儿CH筛查阳性率、阳性预测值、发病率的差异。结果 583 979例新生儿CH筛查中,初筛阳性6 127例,筛查阳性率为1.0%(6 127/583 979),共检出CH患儿425例,阳性预测值为6.9%(425/6 127),发病率为1∶1 374。女性、早产儿和过期产儿、极低出生体重儿和低出生体重儿组的CH筛查阳性预测值和发病率分别高于男性、足月儿和正常体重儿组,差异均有统计学意义(P<0.05)。结论新生儿疾病筛查是CH早期诊断的有效措施之一,女性、早产、过期产和低出生体重是影响新生儿CH发病的风险因素。
Objective To investigate the newborn sex,gestational age,birth weight and congenital hypothyroidism screening efficiency and the incidence of the disease and provide scientific evidence for the prevention of CH and eugenics counseling service.Methods The screening results of congenital hypothyroidism(CH)in Guangzhou Neonatal Disease Screening Center between January 2015 to December 2017 were analyzed retrospectively.To explore the difference of neonatal CH screening positive rate,positive predictive value and incidence rate among different sex,gestational age and birth weight group.Results A total of 583 979 newborns were screened from January 2015 to December 2017.6 127 cases was screening positive and the screening positive rate was 1.0%.425 cases were confirmed positive for CH and the incidence rate was 1∶1 374.The positive predictive value was 6.9%(425/6 127).The positive predictive value and the incidence rate of CH in female infants,preterm infants,post-term infants,very low birth weight infants and low birth weight infants group were respectively higher than those in male infants,full-term infants and normal weight infants group,the differences were statistically significant(P〈0.05).Conclusion Neonatal disease screening is an effective measure for early diagnosis of neonatal CH.Female sex,preterm birth,older gestational age and low birth weight are risk factors affecting the incidence of CH in newborns.
