摘要
目的分析肺开放结合中等潮气量对内毒素急性肺损伤犬的影响及七氟烷的干预作用。方法以40只健康杂种公犬为试验对象,建立ALT模型并随机分为4组,分别给予CNC、RMV+PLV、RMV+PLV+七氟烷以及七氟烷+RMV+PLV通气治疗,给药6 h后,测定各组模型犬血浆中TNF-α、IL-10、IL-6水平并对比;处死模型犬,进行组织病理学观察,判定病变程度进行评分,并比较总分;对比各组模型犬支气管肺泡灌洗液中NF-KB、IL-10、IL-6水平。结果 CNC组血浆中TNF-α、IL-10、IL-6水平,组织病理学病变程度,肺泡灌洗液中NF-KB、IL-10、IL-6水平与其余各组均有所差异,差异有统计学意义(P<0.05);给予七氟烷吸入的两组各指标与CNC组和RMV+PLV组相比,差异有统计学意义(P<0.05),但两组本身对比无统计学意义(P>0.05)。结论肺开放结合中等潮气量,能更好改善肺损伤状况,给予模型犬七氟烷吸入治疗,干预效果更突出,但七氟烷发挥作用时机并无显著差异,需深入研究。
Objective To analyze the effect of open lung combined with moderate tidal volume on endotoxin-induced acute lung injury in dogs and the intervention effect of sevoflurane. Methods 40 healthy mongrel male dogs were chosen as subjects. ALT model was established and subjects were randomly divided into 4 groups,treated with CNC,RMV +PLV,RMV + PLV + sevoflurane and sevoflurane + RMV + PLV ventilation,respectively. 6 hours later,TNF-α,IL-10,IL – 6 levels in plasma of each model dog were measured and compared. The model dogs were then sacrificed for histopathological observation and assessment of the lesion extent and the total score was compared. The levels of NF-KB,IL-10 and IL-6 in bronchopulmonary alveolar lavage fluid were also compared. Results In CNC group,TNF-α,IL-10,IL-6 level in plasma,histopathological lesions extent,NF-KB,IL-10,IL-6 level in alveolar lavage fluid differed from those of the rest of the group,the differences statistically significant( P〈0. 05). Compared with CNC group and RMV + PLV group,the two groups inhaling sevoflurane showed a statistically significant difference( P〈0. 05),but the comparison between the two groups was not statistically significant( P〉0. 05). Conclusion The combination of open lung combined with medium tidal volume better improves the condition of lung injury. If sevoflurane inhalation therapy is given to dogs,the intervention effect is more prominent,but the working time of sevoflurane didn't show any significant difference,which needs further study.
