摘要
目的探讨3.0T多参数MRI(multi-parametric MRI,Mp-MRI)前列腺影像报告和数据系统(prostate imaging reporting and data system version 2,PI-RADS V2)评分对前列腺病变的诊断价值及与穿刺病理的相关性。方法回顾性分析经病理证实的82例前列腺病变患者Mp-MRI资料,其中前列腺癌34例,良性前列腺增生39例、前列腺炎9例;由2名影像医师采用盲法对患者Mp-MRI图像用PI-RADS V2评分并记录结果。采用Spearman相关分析PI-RADS V2评分与穿刺病理的相关性,并利用ROC曲线评估PI-RADS V2评分对前列腺癌的诊断效能。结果 PI-RADS V2评分与穿刺病理呈正相关,r=0.378。PI-RADS V2评分诊断前列腺癌的ROC曲线下面积0.868,敏感性为88.2%,特异性为85.4%,准确性为86.6%。结论 PI-RADS V2评分系统对前列腺癌的诊断和鉴别诊断价值高,PI-RADS V2评分与穿刺病理呈正相关。
Objective To explore the diagnostic value of prostate imaging reporting and data system score version 2 (PI-RADS V2) for prostate disease detection using 3.0 T multi-parametric MRI and the correlation between the score of PI-RADS V2 and ultrasound guided systemic biopsy. Methods Mp-MRI images of eighty-two patients with pathology-confirmed prostate diseases were retrospectively analyzed, thirty-four were confirmed pathologically as prostate cancer, thirty-nine were BPH and nine were prostatitis. The images of Mp-MRI were analyzed by two diagnostic physicians independently with blind method using PI-RADS V2 score and the location of specific lesion per case was denoted. Spearman correlation analysis was used to compare differences between PI-RADS score and pathological findings. The power of diagnosis for prostate cancer using PI-RADS V2 were subjected to receiver operating characteristic (ROC) curve analysis. Results The PI-RADS V2 score was positively correlated with systemic biopsy ( r =0.351, P 〈0.05). The sensitivity, specificity, accuracy, and area under the ROC curve of diagnostic performance of the PI-RADS V2 score were 88.2%, 85.4%%, 86.6% and 0.868. Conclusion The PI-RADS V2 score was positively correlated with systemic biopsy. The use of PI-RADS V2 can improve the diagnostic efficiency significantly for the diagnosis and differential diagnosis of prostate cancer prominently.
