摘要
目的:探讨前列腺液中锌离子、前列腺特异性抗原(PSA)、巨噬细胞炎性蛋白-1α(MIP-1α)与慢性非细菌性前列腺炎患者症状的相关性。方法:选取436例慢性非细菌性前列腺炎患者,其中ⅢA型291例,ⅢB型84例、Ⅳ型61例,并选同期身体健康体检者128例为正常组,收集所有被检者的前列腺液,采用湿玻片法计数前列腺液中白细胞(WBC)、卵磷脂小体(SPL)、化学比色法和酶联法测定锌离子、PSA和MIP-1α水平,采用慢性前列腺炎症状评分量表(CPSI)评估患者的前列腺炎症状,对CPSI评分与前列腺液中锌离子、PSA、MIP-1α、WBC和SPL的关系进行Pearson线性相关性分析。结果:ⅢA型组、Ⅳ型组患者前列腺液中WBC≥1+的比例高于正常组,差异具有统计学意义(P<0.05);ⅢA型组、ⅢB型组患者前列腺液中的锌离子及PSA水平低于Ⅳ型组及正常组,MIP-1α水平高于Ⅳ型组和正常组,差异具有统计学意义(P<0.05);ⅢA型组患者前列腺液中PSA水平低于ⅢB型组,MIP-1α水平高于ⅢB型组,差异具有统计学意义(P<0.05);慢性前列腺炎患者前列腺液中锌离子和PSA浓度与CPSI评分呈负相关性(P<0.05),MIP-1α与CPSI评分呈正相关性(P<0.05)。结论:前列腺液中锌离子、PSA和MIP-1α与慢性非细菌性前列腺患者症状严重程度相关,对慢性非细菌性前列腺炎的诊断和评估具有参考价值。
Objective: To investigate the relevance of zinc ion,prostate specific antigen and chemokine MIP-1α in the diagnosis of chronic nonbacterial prostatitis. Methods: 436 patients of chronic nonbacterial prostatitis were selected,including 291 III A,84 cases of III B and 61 cases of IV,and128 cases of healthy group were selected at the same time. All subjects were collected succus prostaticus,and EPS-WBC and SPL were counted by the wet slides method,and chemical colorimetric method and ELISA method were adopted to measure zinc ions,PSA and MIP-1α in prostatic fluid; adopting CPSI to evaluate patients' prostatitis symptoms; adopting Pearson linear relevance analysis between CPSI scores and zinc ions,PSA and MIP-1α,WBC and SPL in prostatic fluid. Results: WBC≥1 +proportion in prostatic fluid of III A group and IV group was higher than normal group,differences were statistically significant( P〈0. 05); zinc ions and PSA levels in prostatic fluid of III A group and III B group were lower than IV group and normal group,MIP-1α level was higher than IV group and normal group,differences were statistically significant( P〈0. 05); PSA level in prostatic fluid of III A group was lower than III B group,MIP-1α level was higher than IIII B group,differences were statistically significant( P〈0. 05); zinc ions and PSA concentration were negatively correlated with CPSI scores in prostatic fluid of chronic prostatitis patients( P〈0. 05),while MIP1α was positively correlated with CPSI scores( P〈0. 05). Conclusion: zinc ions,PSA and MIP-1α in prostate liquid were closely related with symptom severity of chronic nonbacterial prostate,which are valuable for the diagnosis and evaluation of chronic nonbacterial prostatitis.
