摘要
目的探讨咸阳地区妇产医院分娩产妇孕期营养与出生结局的相关性。方法本研究为回顾性调查,调查对象为2016年1月至2016年4月在咸阳市妇产医院产科分娩的产妇。人口学资料、孕期营养补充情况采用自行设计的问卷调查,出生结局资料通过医院HIS系统跟踪,分别采用多因素线性回归分析法和非条件Logistic回归分析法分析孕期营养与出生结局的关系。结果叶酸+膳食种类数≥25、叶酸+鱼蛋禽畜肉类≥5与两者均否的分娩孕周比较,差异均有统计学意义(P<0.05)。叶酸+膳食种类数≥25与只服叶酸组、两者均否组的新生儿出生体重相比,分别增加65.06 g和172.25 g;叶酸+鱼蛋禽畜肉类≥5与只服叶酸组、两者均否组的新生儿出生体重相比,分别增加49.93 g和130.84 g;差异均具有统计学意义(P<0.05)。叶酸+膳食种类数≥25与只服叶酸比较,妊娠并发症发生率更低(P<0.05);与只服叶酸和两者均否的妊娠并发症发生率比较,叶酸+鱼蛋禽畜肉类≥5妊娠并发症发生率更低(P<0.05);与只服叶酸和两者均否比较,叶酸+鱼蛋禽畜肉类≥5围生儿并发症发生率更低(P<0.05)。结论围孕期在服用叶酸的基础上增加膳食种类数,尤其是鱼、蛋、禽畜肉类的种类数,可以显著改善出生结局。
Objective To analyze the correlation between maternal nutrition and birth outcomes in Xianyang Maternity Hospital. Methods The clinical data of maternal in the obstetrics department of Xianyang Maternity Hospital from January 2016 to April 2016 were retrospectively reviewed. Demographic data and nutritional supplements during pregnancy were investigated by a self-designed questionnaire, and the birth outcomes data were tracked through the HIS. The correlations between maternal nutrition and birth outcomes were analyzed through multivariate linear regression analysis and non-conditional Logistic regression analysis. Results There were statistically significant differences in gestational weeks between those who took folic acid + had a dietary variety≥25, took folic acid + had fish, egg, meat variety ≥5 and those who did neither(P〈0.05). For those who took folic acid + had a dietary variety≥25, compared with those who took only folic acid and those who did neither, birth weight increased by 65.06 g and 172.25 g; and for those who took folic acid + had fish,egg, meat variety ≥5, compared with those who took only folic acid and those who did neither, birth weight increased by49.93 g and 130.84 g, which were all statistically significant(P〈0.05). Compared with those who took only folic acid, the rate of pregnancy complications in those who took folic acid + had a dietary variety ≥25 was lower(P〈0.05). Compared with those who took only folic acid and those who did neither, the rate of pregnancy complications in those who took folic acid + had fish, egg, meat variety ≥5 was lower(P〈0.05). Compared with those who took only folic acid and those who did neither, there were lower rate of prinatal complication in those who ook folic acid + had fish, egg, meat variety ≥5(P〈0.05). Conclusion Women in peri-conception period should take folic acid reasonably, balance diet variety, increase egg,meat, dairy and fish intake, so as to promote the health status of women and perinatal infants.
