摘要
目的分析增强磁共振成像对贝尔面瘫的诊断价值。方法选取重庆市江津区中心医院2013年1月—2016年12月收治的贝尔面瘫103例,按照病程分为发展期组34例,静止期组39例,恢复期组30例。记录3组增强磁共振成像在患侧和健侧的检测结果,比较3组面神经直径及面神经强化程度。结果在常规核共振扫描轴位T1WI上,面瘫患者患侧和健侧的面神经均表现为等信号;在轴位T2WI上,其在患侧和健侧表现为部分等信号、部分略高信号。增强MRI显示3组患侧面神经有所增粗,患侧面神经直径均大于健侧(P<0.05);但组间患侧和健侧的面神经直径比较差异均无统计学意义(P>0.05)。增强MRI显示患侧面神经有节段性异常强化的表现,并以面神经内耳道底段强化、膝状神经节强化和迷路段强化为主要表现,3组患侧面神经强化程度均高于健侧,且静止期组大于进展期组和恢复期组(P<0.05),3组健侧面神经强化程度比较差异无统计学意义(P>0.05)。结论贝尔面瘫后,经增强磁共振成像显示患侧面神经直径有所增加,面神经强化程度增强,且在不同时期表现程度也有所差异。
Objective To analyze value of enhanced magnetic resonance imaging (MRI) in diagnosis of bell palsy. Methods A total of 103 patients with bell palsy admitted during Janury7 2013 and December 2016 were divided into developing stage group (n= 34) , silent period group (n= 39) and recovery stage group (n =30) according to cour- ses of disease. Results of enhanced MRI in both affected and uninjured sides were recorded, and then results of facial nerve diameter and facial nerve enhancement degree were compared in three groups. Results In all patients, facial nerve in both affected and uninjured sides showed equal signals on conventional MRI scan axial T1WI; facial nerve in both affected and uninjured sides showed partial equal signals and slightly higher signal in other part on axial T2WI. En- hanced MRI showed that facial nerves of affected side were thickening, and facial nerve diameters of affected sides were greater than those of uninjured sides in three groups (P 〈 0. 05) ; there was no significant difference in diameter between affected and uninjured sides in the same group (P 〉0.05). Enhanced MRI showed that abnorlaal segmental enhance- laent was found in facial nerve of affected side, and main manifestations were enhancement of the basal segment of inter- nal auditory canal in facial nerve, enhancement of the geniculate ganglion and enhancement of labyrinthine segment. De- grees of facial nerve enhancement in affected sides were greater than those in uninjured sides among three groups, and the degree in silent period group was significantly greater than those in developing stage and recovery stage groups (P 〈 0. 05 ). There were no significant differences in degree of facial nerve enhancement in uninjured sides among three groups (P 〉 0. 05). Conclusion Enhanced magnetic resonance imaging shows that increased diameter of facial nerve enhance- ment in affected side, enhanced degree of facial nerve enhancement in patients with bell palsy, and the manifestations are different in different periods.
