摘要
目的研究磁共振弥散加权成像对急性脑梗死诊断的意义及效果。方法选取云南省第二人民医院于2016年6月至12月收治的66例急性脑梗死患者,对所有病例首先行常规MRI检查,之后再行磁共振弥散加权成像检查,二者检查时间间隔1 h之上,然后对所有患者病灶区的ADC值进行计算和分析比较。结果磁共振弥散加权成像对超急性和急性脑梗死患者高信号显示率能达到100%,而传统MRI检查则显示为高信号或等信号。经过MRI和磁共振弥散加权成像检查对比之后发现,超急性期和急性期脑梗死患者ADC值均低于对侧正常脑组织,两者相比具有统计学意义(P<0.05),且急性期脑梗死患者ADC值高于超急性期患者。结论磁共振弥散加权成像对急性脑梗死患者的诊断相对于常规MRI具有更高的灵敏性,对急性脑梗死患者的诊断和临床治疗具有更高的指导意义,对不同时期梗死病灶变化具有鉴定意义,可为急性脑梗死患者的有效个体化治疗提供依据。
Objective To study the significance and effects of diffusion-weighted imaging in the diagnosis of acute cere-bral infarction. Methods Sixty-six cases of patients with acute cerebral infarction treated in the Second People's Hospital of Yun-nan between June 2016 and December 2016 were selected. All the patients were given routine MRI examination, and, after a one-hour interval, they underwent diffusion-weighted imaging examination. The ADC values in the lesions of all the patients with a-cute cerebral infarction were calculated, analyzed and compared. Results Diffusion-weighted imaging achieved a high signal dis-play rate of 100% in patients with hyperacute or acute cerebral infarction while conventional MRI examination showed high sig-nals or equal signals only. After MRI and diffusion-weighted imaging, the ADC values of patients with hyperacute or acute cere-bral infarction were lower than those of contralateral normal brain tissues (P〈0. 05), and the ADC value in patients with acute cerebral infarction was higher than that in patients with hyperacute cerebral infarction. Conclusion Diffusion-weighted imaging has higher sensitivity than routine MRI in the diagnosis of acute cerebral infarction. The former approach can serve as a better guide to the diagnosis and clinical treatment of patients with acute cerebral infarction, help detect the changes of infarction lesions at different stages and contribute to individualized treatment of patients with acute cerebral infarction.
