血清降钙素原在感染性脑膜炎中的诊断价值

摘要 目的分析血清降钙素原在感染性脑膜炎诊断中的应用价值。方法选取120例感染性脑膜炎患儿,其中病毒性脑膜炎54例,细菌性脑膜炎66例,选取同期体检健康的60名儿童为对照组,比较3组受试者血清降钙素原情况。结果细菌性脑膜炎C-反应蛋白(88.54±12.65)mg/L、血清降钙素原(5.15±5.87)ng/L、白细胞计数水平(15.57±1.18)×109/L均明显高于病毒性脑膜炎患儿的(16.67±2.31)mg/L、(0.24±0.12)ng/L、(10.45±1.06)×10~9/L及对照组的(1.32±0.85)mg/L、(0.05±0.13)ng/L、(6.35±1.02)×10~9/L(P<0.05);病毒性脑膜炎患儿C-反应蛋白、白细胞计数水平均明显高于对照组(P<0.05),血清降钙素原与对照组比较差异无统计学意义(P>0.05)。血清降钙素原对感染性脑膜炎诊断的敏感度95.45%,特异度96.30%,准确度95.83%。结论血清降钙素原在鉴别诊断细菌性脑膜炎与病毒性脑膜炎中价值显著,然而其在病毒性脑膜炎患儿中血清降钙素原的升高有限。 Objective To analyze the application value of serum procalcitonin in the infectious meningitis. Methods120 cases of children with infectious meningitis including 54 cases with virus meningitis and 66 cases with bacterial meningitis were selected, and 60 cases of children with physical examination at the same period were selected as the control group, and the serum procalcitonin was compared between the three groups. Results The bacterial meningitis C-reactive protein(88.54±12.65)mg/L, serum procalcitonin(5.15±5.87)ng/L, white blood cell count(15.57±1.18)× 10~9/L were obviously higher than those of children with virus meningitis, [(16.67±2.31)mg/L,(0.24±0.12)ng/L,(10.45±1.06)×10~9/L] and those in the control group, [(1.32±0.85)mg/L,(0.05±0.13)ng/L,(6.35±1.02)×10~9/L](P0.05), and the C-reactive protein and leucocyte count level in the children with virus meningitis was obviously higher that in the control group(P0.05), and the difference in the serum procalcitonin between the two groups was not obvious(P0.05), and the sensitivity, specificity and accuracy of serum procalcitonin to the infectious meningitis were respectively 95.45%,96.30%, 95.83%. Conclusion The value of serum procalcitonin in differential diagnosis of bacterial meningitis and infectious meningitis is obvious, but the increase of serum procalcitonin in the children with virus meningitis is limited.
