摘要
目的探讨磁性酶联免疫分析法和流式细胞术两种方法检测HLA-B27的临床价值,为临床血液HLA-B27的检验提供科学理论依据。方法方便选取该院2017年1—12月在该院检测血液HLA-B27的134例患者为研究对象,所有患者均采用两种方法进行检验,根据检验方法不同,分为对照组和观察组。对照组采用流式细胞仪检验,观察组采用磁性酶联免疫分析法进行检验,观察两种检验方法的灵敏度、特异性及阳性、阴性预测值。结果磁性酶联免疫分析的灵敏度、特异性及阳性、阴性预测值分别为0.961,0.985,0.981,0.985,流式细胞仪的灵敏度、特异性及阳性、阴性预测值分别为0.961,0.971,0.968,0.973,两种方法结果的符合率为97.76%,差异无统计学意义(P>0.05)。结论磁性酶联免疫分析法在检验血液HLA-B27的灵敏度和特异性较好,具有方面、省时、简单、准确的特点,适合强直性脊柱炎的早期诊断及鉴别,实用可靠,值得在临床上应用推广。
Objective This paper tries to explore the clinical value of magnetic enzyme-linked immunosorbent assay and flow cytometry in the detection of HLA-B27, and to provide scientific theoretical basis for the clinical blood HLA-B27 test.Methods 134 patients with HLA-B27 in the hospital from January to December 2017 were selected conveniently as the study subjects. All patients were tested by two methods. According to the different testing methods, they were divided into control group and observation group. The control group was examined by flow cytometry. The observation group was tested by magnetic enzyme-linked immunosorbent assay. The sensitivity, specificity, and positive and negative predictive value of the two test methods were observed. Results The sensitivity, specificity, and positive and negative predictive values of magnetic enzyme-linked immunosorbent assay were 0.961,0.985,0.981 and 0.985, respectively. The sensitivity, specificity, and positive and negative predictive values of flow cytometry were 0.961,0.971,0.968,0.973. The coincidence rate of the two methods was 97.76%. the difference was not statistically significant. Conclusion The magnetic enzyme-linked immunosorbent assay has good sensitivity and specificity in the detection of blood HLA-B27. It has the characteristics of being timesaving, simple, and accurate. It is suitable for the early diagnosis and identification of ankylosing spondylitis. It is practical and reliable, and worthy of clinical application.
