2006—2017年福建泉州市流感病原特征及检出率影响因素 被引量:7

Flu pathogen characteristics and factors affecting detection rate in Quanzhou City(2006-2017)

摘要 目的掌握泉州市流感样病例(ILI)病原特征及检出率影响因素,为监测和防控提供依据。方法通过国家流感监测网络,收集2006—2017年泉州市哨点医院和聚集性疫情的ILI咽拭子标本,采集病例基本信息,用荧光定量RT-PCR法进行核酸检测,用Logistic回归分析检出率的影响因素。结果 2006—2008年B型是优势株,2009—2017年B型、季节性H3与新甲型H1N1交替流行;6~15岁检出率最高;咽拭子核酸阳性检出率18.79%(3 240/17 241);以B型、季节性H3和新甲型H1N1型为主;年龄、季节、标本来源、采集时间与送检时间间隔,对检出率影响差异有统计学意义(P<0.05)。结论泉州市冬春季和夏季是流感病毒活动高峰,来源于暴发疫情的阳性率高于国家级流感样病例监测哨点医院,采集时间与送检时间间隔越大,阳性率越低,应加强泉州市冬春季节和夏季对于小学生或者初中生暴发疫情的监测,加强监督哨点医院采集样本之后及时送检。 Abstract: Objective To grasp the etiology characteristics and influence factors of influenza in Quanzhou, in order to provide a scientific basis for prevention and control of influenza. Methods Through the National influenza Surveillance Network System, the throat swab specimens of influenza like illness fi'om Quanzhou National influenza Surveillance Sentinel Hospitals and influenza outbreaks during 2006 -2017 were collected, the basic information of cases were collected at the same time. Specimens nucleic acids were tested by fluorescence quantitative RT-PCR. Logistic regression analysis was used to analyze the influence factors. Results From 2006 to 2008, influenza B virus was the dominant epidemic strains, during 2009 -2017, influenza B virus,seasonal A H3 virus and influenza H1N1 occurred alternately. 6 = 15 age group had the highest positive rate of influenza virus, pharyngeal swab positive rate nucleic acid was 18.79% (3 240/17 241 ). influenza B virus, seasonal A H3 virus and influenza HIN1 virus accounted for the main components. Age,season,specimen source,the duration from sample collection to detection were the major factors influencing the positive rate. Conclusion In Quanzhou, winter, spring and summer showed a seasonal peak,the positive rate of outbreak was higher than that from the National influenza Surveillance Sentinel Hospitals. The longer the interval between the collection time and the inspection time, the lower the positive rate. The surveillance for the primary and junior middle school students during winter, spring and summer should be strengthened, and the sending for detection from the sentinel hospitals should be in time.
