摘要
目的探讨二甲双胍治疗多囊卵巢综合征(PCOS)的疗效及其对甲状腺功能的影响,为PCOS的临床治疗提供参考。方法纳入我院2015年7月至2016年10月收治的80例甲状腺功能正常的PCOS患者,患者均口服二甲双胍500 mg/次,3次/d。疗程为3个月。比较治疗前、后PCOS患者空腹血糖(FSG)、空腹胰岛素(FINS)、体重指数(BMI)、腰臀比(WHR)、促卵泡素(FSH)、促黄体生成素(LH)、雌二醇(E_2)、睾酮(T)、促甲状腺激素(TSH)、游离甲状腺素(FT_4),记录治疗结果及药物不良反应。结果治疗3个月后,80例患者中,18例患者(22.50%)恢复排卵月经,35例患者(43.75%)月经恢复正常,10例(12.50%)患者成功妊娠。治疗后,患者WHR、FINS、LH、T、TSH水平显著低于治疗前,BMI显著高于治疗前,差异均有统计学意义(P<0.05);患者治疗前、后FSG、FSH、E_2、FT_4水平比较,差异无统计学意义(P>0.05)。患者治疗期间无明显不良反应,停药或剂量减半后恢复正常。结论二甲双胍治疗PCOS可抑制患者TSH,不影响患者FT_4水平。
Objective To investigate the effect of metformin on thyroid function in patients with polycystic ovary syndrome（PCOS）, and provide the reference for treatment of PCOS. Methods From July 2015 to October 2016, a total of 80 PCOS patients who with normal thyroid function in our hospital were enrolled. All the patients received metformin 500 mg per time,3 times per day. The course of treatment was 3 months. The fasting glucose（FSG）, fasting insulin（FINS）, body mass index（BMI）, waist-to-hip ratio（WHR）, follicle stimulating hormone（FSH）, luteinizing hormone（LH）, estradiol（E2）, testosterone（T）,thyroid stimulating hormone（TSH） and free thyroxine（FT4） in PCOS patients before and after treatment were compared, and the treatment results and adverse drug reactions were recorded. Results After 3 months of treatment, of the 80 patients, 18 patients（22.50%） resumed ovulation menstruation, 35 patients（43.75%） returned to normal menses, and 10 patients（12.50%）succeeded in pregnancy. After treatment, the WHR, FINS, LH, T and TSH levels were significantly lower than those before treatment, and the BMI was significantly higher than that before treatment（P〈0.05）. There were no statistical significances in FSG, FSH, E2, FT4 levels before and after treatment（P〉0.05）. There was no obvious adverse reaction during treatment, and it returned to normal after withdrawal drug or dose reduction. Conclusion Metformin can inhibit the TSH in PCOS patients without affecting the level of FT4.
