右美托咪定复合星状神经节阻滞对气管插管反应的影响 被引量:1

摘要 目的观察右美托咪定(Dex)联合星状神经节阻滞(SGB)对于气管插管反应的影响。方法选取全麻手术患者160例,随机分为对照组(C组)、右美托咪定组(D组)、星状神经节阻滞组(S组)、右美托咪定联合星状神经节阻滞组(DS组),每组40例。C组:诱导前无任何处理,D组:诱导前Dex静脉泵入,S组:诱导前行右侧SGB,DS组:先行SGB,再静脉泵入Dex。记录麻醉前(T0)、插管时(T1)、插管后1 min(T2)、3 min(T3)的收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、心率(HR)的变化。在4个时点分别抽取患者静脉血,测量血糖和皮质醇。结果 SBP和DBP:与T0相比,C组均升高(P<0.05);相同时刻,与C组相比,其余3组均降低(P<0.05),DS组显著下降(P<0.01)。MAP:与T0相比,C组、D组、S组均升高(P<0.05);相同时刻,与C组相比,其余3组均降低(P<0.05),DS下降明显(P<0.05)。HR:与T0相比,C组升高(P<0.05);相同时刻,与C组相比,其余3组均降低(P<0.05),DS下降明显(P<0.05)。血糖和皮质醇:与T0相比,C组、D组、S组各时间点均升高(P<0.05);相同时刻比较,DS组均显著低于其余3组(P<0.05)。结论右美托咪定联合星状神经节阻滞可以降低全麻患者的气管插管反应,从而使全麻诱导更加平稳。 Objective To observe the effect of dexmedetomidine combined with stellate ganglion block on patients with tracheal intubation response. Methods Totally 160 patients undergoing general anesthesia were randomly divided into four groups: control group(group C), Dex group(group D), SGB group(group S), Dex combined with SGB group(group DS), 40 cases in each group. Group C: no treatment before induction. Group D: Dex administrated before induction. Group S: SGB on the right side was performed before induction. Group DS: SGB combined with Dex. SBP,DBP, MBP and HR were recorded before anesthesia induction(T0), intubation immediately(T1), 1 minute after intubation(T2), 3 minutes after intubation(T3). Blood glucose and concentrations of cortislo were detected at four different time points. Results SBP and DBP: Compared with T0, they were significantly increased in group C(P0.05). Compared with group C, they were lower in the other three groups at the same time point(P0.05), group DS significantly decreased(P0.01). MAP: Compared with T0, they were significantly increased in group C, D, S(P0.05). Compared with group C, they were lower in the other three groups at the same time point(P0.05), the MAP in group DS was lowest(P0.05). HR:Compared with T0, it was higher in group C(P0.05). Compared with group C, they were lower in the other three groups at the same time point(P0.05), the HR in group DS was lowest(P0.05). Compared with T0, the blood glucose and concentrations of cortislo were higher in group C, D and S(P0.05). At the same time point, they in group DS were lower than that in the other three groups(P0.05). Conclusion Dexcombined with right-side stellate ganglion block can significantly decrease the cardiovascular response to tracheal intubation, so general anesthesia revulsion steadily proceeds.
