摘要
目的探讨脑脊液中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及神经元烯醇化酶(NSE)在小儿中枢神经系统感染疾病(CNSI)中的诊断价值。方法选取2015年10月至2017年1月该院收治的54例CNSI住院患儿,将其分为病毒性脑膜炎组(病脑组30例)和化脓性脑膜炎组(化脑组24例)。另选取同期入院的经脑脊液等相关检查患儿20例作为对照组。采用酶联免疫吸附试验(ELISA)检测3组患儿脑脊液中TNF-α、IL-6及NSE 3种生物标记物的水平。结果 3组患儿脑脊液中TNF-α、IL-6及NSE水平均为:化脑组>病脑组>对照组。化脑组患儿脑脊液中的TNF-α、IL-6及NSE水平高于病脑组及对照组,3组比较,差异均有统计学意义(P<0.05),但数据有较多的重叠,而病脑组与对照组TNF-α、IL-6及NSE水平比较,差异均无统计学意义(P>0.05)。结论脑脊液中TNF-α、IL-6及NSE水平检测对CNSI的诊断具有一定临床应用价值,但需进一步采用精确的检测方法进行大样本临床试验验证。
Objective To explore the diagnostic value of tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6)and neuron enolase(NSE)in the cerebrospinal fluid of children with central nervous system infection(CNSI).Methods 54 cases of CNSI hospitalized children,admitted in the hospital from October2015 to January 2017,were enrolled in the study and divided into viral meningitis group(30 cases)and suppurative meningitis group(24 cases).Another 20 cases who underwent cerebrospinal fluid examination and other related examinations were enrolled in the study as the control group.The levels of three biomarkers TNF-α,IL-6 and NSE in cerebrospinal fluid of three groups were detected by enzyme linked immunosorbent assay(ELISA).Results The levels of TNF-α,IL-6 and NSE in purulent meningitis group were the hightest,and the levels of these three factors in viral meningitis group were highter than the control group,and the difference was statistically significant in three groups(P〈0.05).But there were a lot of data overlaps.There was no significant difference in the levels of TNF-α,IL-6 and NSE between the brain group and the control group(P〈0.05).Conclusion The detection of TNF-α,IL-6 and NSE in cerebrospinal fluid has a certain clinical value for the diagnosis of CNSI,but it needs to be further verified by a large sample clinical trial.
