摘要
目的观察血管内介入治疗内镜下止血失败的动脉性消化道出血及异常血管病变效果。方法将60例血管内介入治疗内镜下止血失败的动脉性消化道出血患者根据计算机表法分为观察组和对照组各30例,两组患者均有明确的直接出血表现,对照组采用栓塞治疗,观察组采用留置导管泵药治疗。比较两组患者出血停止时间和其他异常血管病变的发生率。结果两组出血停止时间分别为(1.34±0.15)d和(1.42±0.18)d,治疗后其他异常血管病变的发生率均分别为10.00%(3/30)和6.67%(2/30),组间差异均无统计学意义,P>0.05。结论栓塞治疗和血管内介入治疗对内镜下止血失败的动脉性消化道出血患者均有较好的临床止血效果,且两种血管内介入治疗方案的异常血管病变发生率均较低,临床可根据具体情况选择治疗方案。
Objective To observe the effect of intravascular interventional therapy on arterial gastrointestinal bleeding and abnormal vascular disease which failed to stop bleeding under endoscopy. Methods Sixty patients with arterial gastrointestinal bleeding who failed to stop bleeding under endoscopy in intravascular interventional therapy were divided into observation group and control group according to computer table method, with 30 cases in each group. Both groups showed clear direct bleeding, the control group was treated with embolization, and the observation group was treated with indwelling catheter pump. The bleeding stop time and incidence of other abnormal vascular diseases were compared between the two groups. Results The stopping time of bleeding in the two groups was(1.34±0.15) d and(1.42±0.18) d respectively, and the incidence of other abnormal vascular diseases were 10.00 %(3/30) and 6.67 %(2/30) respectively after treatment, and there was no statistical difference between the two groups, P〉0.05. Conclusion Embolization therapy and intravascular interventional therapy have good clinical hemostatic effect in patients with arterial gastrointestinal bleeding who fail to stop bleeding under endoscopy, and the incidence of abnormal vascular lesions in both intravascular interventional therapy schemes is low, so clinical treatment schemes can be selected according to specific situations.
