摘要
目的:探讨磁共振高分辨率睾丸成像的方法。方法:随机抽取30名20~56岁健康男性志愿者。实验组30例:在检查部位垫衬超声耦合剂软胶袋;对照组30例:检查部位不做任何检查前处理。实验组与对照组为同一组志愿者,前后进行两次同序列、同参数高分辨率扫描。测试序列:轴面T_1WI、T_2WI、T_2WI+FS、矢状面和冠状面T_2WI+FS。计算每一幅图像的SNR、CNR值,并对各序列成像质量评分,将所得数据进行统计学分析,比较两组间影像质量的差异。结果:实验组影像质量明显优于对照组。客观评估:轴面T_1WI、T_2WI、T_2WI+FS的SNR、CNR均值分别是SNR实验组:4.03±0.71、7.28±3.61、4.22±2.10,对照组:3.58±0.62、3.44±2.24、2.38±1.78;CNR实验组:0.71±0.24、1.16±0.66、0.56±0.25,对照组:0.57±0.21、0.53±0.44、0.20±0.13,两组间各序列差异均存在统计学意义,SNR的t值分别是-2.746、-5.595、-3.663,P值均<0.05,CNR的t值分别是-2.760、-4.590、-8.469,P值均<0.05。矢、冠状面T_2WI+FS的SNR均值分别是实验组:5.97±2.00、7.58±3.04,对照组:5.20±1.79、6.81±2.67,两组间各序列差异不存在统计学意义,t值分别是-1.964、-1.872,P值均>0.05;CNR均值分别是实验组:3.18±1.13、2.88±0.96,对照组:2.44±1.08、2.40±0.75,两组间各序列间差异存在统计学意义,t值分别是-2.766、-2.676,P值均<0.05。主观评分:两位医师分别对两组测试序列成像质量评分的一致性较好,Kappa值分别是0.72、0.71;0.80、0.77;0.84、0.79;0.72、0.83;0.85、0.81,95%置信区间分别是0.57~0.83、0.57~0.83;0.66~0.90、0.65~0.87;0.72~0.94、0.68~0.89;0.57~0.85、0.73~0.92;0.72~0.95、0.70~0.90。甲、乙两位医师两组间各序列成像质量评分差异均存在统计学意义,医师甲评分均值分别是实验组:3.38±0.53、3.57±0.50、3.35±0.54、3.24±0.56、3.65±0.54;对照组:2.70±0.66、2.58±0.67、2.26±0.75、2.23±0.79、2.27±0.64,Z值分别是-6.764a、-7.053a、-7.550a、-7.078a、-8.088a,P值均<0.05。医师乙评分均值分别是实验组:3.36±0.52、3.53±0.50、3.39±0.55、3.25±0.50、3.64±0.54,对照组:2.59±0.64、2.66±0.76、2.23±0.75、2.26±0.76、2.24±0.69,Z值分别是-6.529a、-6.539a、-7.894a、-7.319a、-8.051a,P值均<0.05。结论:超声耦合剂软胶袋垫衬法能够明显提高磁共振高分辨率睾丸成像质量。
Objective:To investigate the method of high-resolution testicular magnetic resonance imaging(MRI).Methods:Thirty healthy male volunteers at the age of 20~56 were randomly selected.Each volunteer was scanned two times with the same sequence and parameters with or without the ultrasonic couplant.The study group(30 cases)used ultrasonic couplant in the soft polybag while the control group(30 cases)did not use it during the examination.MRI tests included axial T_1WI,T_2WI,T_2WI+FS,sagittal and coronal T_2WI+FS.The SNR and CNR values of each image were calculated,and the imaging quality scores of each sequence were analyzed.The difference of SNR and CNR were compared between the two groups.Results:The image quality of the study group was better than that of the control group.Objective assessment:The mean values of axial T_1WI,T_2WI and T_2WI+FS(SNR experimental group:4.03±0.71,7.28±3.61,4.22±2.10,control group:3.58±0.62,3.44±2.24,2.38±1.78;CNR:experimental group:0.71±0.24,1.16±0.66,0.56±0.25,control group:0.57±0.21,0.53±0.44,0.20±0.13).There was statistical significant difference in the sequence between groups,SNRt:-2.746,-5.595,-3.66,P〈0.05,CNRt:-2.760,-4.590,-8.469,P〈0.05.The SNR mean values of sagittal and coronal T_2WI+FS(dtudy group:5.97±2.00 and 7.58±3.04;control group:5.20±1.79,6.81±2.67).There was no statistically significant difference in the sequence between groups.The t value of study and control group:SNR(-1.964 and-1.872),P〉0.05.The mean values of CNR:study group:3.18±1.13,2.88±0.96;control group:2.44±1.08,2.40±0.75.There was statistically significant difference in CNR between groups.The t value of study and control group:CNR(-2.766,-2.676),P〈0.05.Subjective score:two physicians respectively had good consistency in two groups of test sequences,Kappa value:0.72,0.71,0.80,0.77,0.84,0.79,0.72,0.83,0.85,0.81;95% confidence interval(0.57~0.83,0.57~0.83,0.66~0.90,0.65~0.87,0.72~0.94,0.68~0.89,0.57~0.85,0.73~0.92,0.72~0.95,0.70~0.90);There was statistically significant difference in the quality score of the sequences between groups by physicians A and B.A's score:Experimental group 3.38±0.53,3.57±0.50,3.35±0.54,3.24 ±0.56,3.65±0.54;Control group:2.70±0.66,2.58±0.67,2.26±0.75,2.23±0.79,2.27±0.64),Z:-6.764 a,-7.053 a,-7.550 a,-7.078 a,-8.088 a,P〈0.05.B's score:Experimental group 3.36±0.52,3.53±0.50,3.39±0.55,3.25±0.50,3.64±0.54;Control group:2.59±0.64,2.66±0.76,2.23±0.75,2.26±0.76,2.24±0.69,Z:-6.529 a,-6.539 a,-7.894 a,-7.319 a,-8.051 a,P〈0.05.Conclusion:Ultrasonic couplant method in the soft polybag can significantly improve the quality of high-resolution MRI of the testis.
