磁共振SWI序列在脑微出血与脑白质病变关系的应用研究 被引量:4

APPLICATION OF SWI IN PATIENTS RELATIONSHIP BETWEEN CEREBRAL MICROBLEEDS AND WHITE MATTER LESIONS

摘要 目的探讨SWI在CMBs诊断以及与WML相关性的应用价值,为脑血管病患者的个体化治疗提供更为可靠的影像学依据。方法对76例脑白质病变患者行头部核磁共振SWI及行头颅常规序列。按照Mantyla对WML进行分级,根据CMBs病灶数量评估分析CMBs与WML的关系。结果随着WML分级的增高CMBs的检出率有上升的趋势(χ2=14.16,P=0.003),分别由23.1%升高至80.0%,尤其2~3级患者CMBs的检出率升高显著;CMBs的检出率与WML的严重程度呈相关性。结论 WML的严重程度与CMBs的发生密切相关,SWI能显著提高CMBs的检出率,对预防脑血管病具有重要意义。 Objective To explore the value of SWI in the diagnosis of CMBs and its correlation with WML,we can provide more reliable evidence for individual treatment of our patients with cerebral vascular disease.Methods 76 patients with white matter lesions from May 2013 to October 2017 in our hospital were collected.They were evaluated with the presence of CMBs and WML using SWI and Routine sequence imaging examination.The WML were scored by Mantyla.The relationship between CMBs and WML was analyzed by the number of CMBs.Results As the score of WML increased,the detection rate of CMBs showed an upward trend(χ2=14.16,P =0.003),increased from 23.1%in patients with the severity of WML=0 to 70.3%and 80.0%in patients with the severity of WML=2 and WML=3;Deep CMBs were closely related to severity of peri-ventricular and deep WML.Conclusion Severity grade of White Matter Lesions and cerebral microbleeds counts are positively related,and SWI can significantly improve the detection rate of CMBs,which is of great significance to the prevention of cerebrovascular disease.
