摘要
目的分析医院自实施医联体后医疗质量改进情况,为进一步提高医疗质量提供参考。方法根据2015年1月1日-2017年12月31日医院实施医联体后各科室的医疗质量监测数据,运用Eecel2007和SPASS3.0软件对医院医疗质量监测数据进行统计分析。结果医院实施医联体后,死亡患者例数累计97例,由40例减少至19例。有2例警训事件,不良事件上报例数由38例增至82例。2015年1月1日-2017年12月31日医院医疗质量管理情况统计结果显示,甲级病案率由92.09%上升至95.21%,三日确诊率由96.50%增至96.88%,手卫生依从率从69%增长至81.9%,增长了18.7%。医院手术管理情况统计显示,累计住院手术总例数为13 499例,三级以上手术例数为6269,术中大量输血例数为0,非计划在手术发生率由2.32‰降至1.91‰,术后感染率分别为2.03‰、3.02‰、2.95‰,2016年和2017年的术后感染率较2015年均有所增加。结论该院自实施医联体后,由于技术精湛、责任心强的专业技术人才的加入,医疗资源利用情况明显提升,虽然加强了医疗质量管理和手术管理,但随着手术量和三、四级手术的增多,医疗风险也在提高,需要我们通过严格落实核心制度,加强质量安全教育等方式来提高医院医疗风险防控能力。
Objective To analyze the hospital medical quality improvement after the implementation of couplet of medical situation, provide reference to further improve the quality of medical treatment. Methods According to January 1, 2015 to December 31, 2017 hospitals after the implementation of couplet of medical medical quality monitoring data of each department use Excel 2007 and SPSS 3.0 software for hospital medical quality monitoring data for statistical analysis. Results After the hospital implemented the medical union, 97 patients died, from 40 to 19. There were 2 police training incidents, and the number of reported adverse events increased from 38 to 82. On January 1, 2015 to December 31, 2017 hospital medical quality management statistics, according to the results of class a medical record rate rose to 92.09% from 95.21%, 3 confirmed rate increased from 96.50% to 96.88%, hand hygiene compliance rate increased to 81.9% from 69%, increased by 18.7%. Hospital operation management, statistics show the accumulative total hospital surgery for 13499 cases, more than 3 surgery for 6269 cases, cases intraoperative massive blood transfusion is 0, the plan on the operating rate decreased from 2.32%0 to 2.32%0, postoperative infection were 2.03%0, 3.02%0, 2.95%0, postoperative infection rate of 2016 and 2017, 2015 annual increase. Conclusion The hospital after the implementation of couplet of medical, because the skilled professional and technical personnel to join, a strong sense of responsibility, health resource utilization is improved significantly, while strengthening the management of medical quality management and operation, but along with the increase in surgery and he third and fourth classes, medical risk also have improved, we need to through the strict implementation of the core system, the patient safety education to improve hospital medical risk control ability.
