摘要
目的探讨综合营养干预对妊娠期糖尿病患者血清脂肪因子及血糖代谢指标的影响。方法选取2016年9月至2017年3月北京亚运村美中宜和妇儿医院收治的90例妊娠期糖尿病孕妇,随机分为常规组(采取常规营养干预治疗)及研究组(采取综合营养干预治疗),两组各45例,并同期选择45例健康孕妇为对照组,对照组孕妇未做任何干预。比较三组血清脂肪因子含量和与血糖代谢指标的水平,并评估常规组与研究组患者干预治疗的效果。结果研究组与常规组在干预前的血清抵抗素、瘦素及视黄醇结合蛋白4水平较对照组均明显升高(t值分别为3.24、2.96、3.75,均P<0.05),且血清中的内脂素与脂联素水平较对照组均明显降低,差异均有统计学意义(t值分别为3.74、3.72,均P<0.05)。研究组患者在干预后的血清抵抗素、瘦素及视黄醇结合蛋白4水平均较常规组低(t值分别为3.53、3.05、3.14,均P<0.05),且血清中的内脂素与脂联素水平较常规组均明显升高,差异均有统计学意义(t值分别为2.97、3.43,均P<0.05),同时其胰岛素抵抗指数、空腹血糖与餐后血糖水平较常规组均明显降低,差异均有统计学意义(t值分别为3.05、2.88、3.13,均P<0.05)。结论妊娠期糖尿病患者血清脂肪因子含量与血糖代谢发生异常,给予综合营养干预治疗能够有效改善其血清脂肪因子含量和血糖代谢指标的情况。
Objective To investigate the effects of comprehensive nutrition intervention on serum lipid factors and blood glucose metabolism indexes in patients with gestational diabetes mellitus.Methods Altogether 90 patients with gestational diabetes mellitus admitted in Beijing Asian Sports Village Amcare Women＇s and Children＇s Hospital from September 2016 to March 2017 were selected and randomly divided into convention group which received routine nutritional intervention treatment and study group which underwent comprehensive nutritional intervention treatment,with 45 cases in each group.And 45 cases of healthy pregnant women were selected at same period as control group,which were not given any intervention.Levels of serum lipid factors and blood glucose metabolism indexes were compared among three groups,and effects of intervention treatment in convention group and study group were evaluated.Results Levels of serum resistin,leptin and retinol binding protein 4 in study group and convention group before intervention were significantly higher than those in control group（t value was 3.24,2.96 and 3.75 respectively,all P〈0.05）,and their serum visfatin and adiponectin levels were significantly lower than those in control group（t value was 3.74 and 3.72 respectively,both P〈0.05）.Levels of serum resistin,leptin and retinol binding protein 4 in study group after intervention were lower than those in convention group（t value was 3.53,3.05 and 3.14 respectively,all P〈0.05）,and serum visfatin and adiponectin levels were significantly higher than those in convention group（t value was 2.97 and 3.43 respectively,both P〈0.05）,and insulin resistance index,fasting blood glucose and postprandial blood glucose levels in study group decreased more significantly than those in convention group（t value was 3.05,2.88 and 3.13 respectively,all P〈0.05）.Conclusion Levels of serum lipid factors and blood glucose metabolism are abnormal in patients with gestational diabetes mellitus.Comprehensive nutrition intervention can effectively improve content of serum lipid factors and blood glucose metabolism indexes.
