苦碟子注射液联合甲钴胺治疗糖尿病周围神经病变临床研究 被引量:13

Kudiezi Injection Combined Mecobalamine for Treatment of Patients with Diabetic Peripheral Neuropathy

摘要 目的:探究苦碟子注射液联合甲钴胺对治疗糖尿病周围神经病变患者的临床研究。方法:选取糖尿病周围神经病变患者70例,采用随机数字表随机分为两组,每组各35人,其中对照组患者在常规降糖治疗的同时,给予甲钴胺治疗,实验组则在常规治疗的基础上,应用苦碟子注射液联合甲钴胺进行治疗。两组均治疗4周。治疗结束后检测治疗前后患者血糖、血脂各项指标的改善情况,以及血液流变学、神经传导速度变化,并计算临床有效率。结果:①分别于治疗前后检测患者正中神经、腓总神经传导速度,结果显示,相比于对照组有明显变化,实验组患者神经功能传导速度上升更加明显,两组结果均有统计学意义(P〈0.05);②检测治疗前后两组患者血脂水平变化情况,其中甘油三酯(TG)、低密度脂蛋白胆固醇(LDL—C)均有明显下降,高密度脂蛋白胆固醇(HDL—C)均有明显上升,实验组变化程度较对照组显著,差异有统计学意义(P〈0.05),而血清总胆固醇(TC)均无明显变化,差异无统计学意义(P〉0.05);③检测治疗前后两组患者空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HBA,C)等糖代谢指标变化,结果显示实验组患者各指标有明显下降,下降程度较对照组有显著性差异,两组结果均有统计学意义(P〈0.05);④治疗结束后,检测两组患者血液流变学指标,实验组和对照组指标均有所下降,但实验组下降更为显著,差异有统计学意义(P〈0.05);⑤治疗结束后,根据临床总有效率监测结果显示,实验组(82.85%)有效率高于对照组(68.57%),两组患者症状改善明显,差异有统计学意义(P〈0.05)。结论:苦碟子注射液联合甲钴胺可改善糖尿病周围神经病变患者血糖、血脂指标,改善神经功能,提高周围神经兴奋性,对糖尿病周围神经病变有显著影响,值得-I盏床推广。 Objective: To explore the effect of Kudiezi Injection combined mecobalamine for the treatment of patients with diabetic peripheral neuropathy. Methods : Seventy patients with diabetic peripheral neuropathy, by the use of random number tables,were randomly divided into two groups, each group 35 cases. The control group' s patients were treated with routine hypoglycemic therapy and at the same time, were given mecobalamine. The treatment group on the basis of routine hypoglycemic treatment, combined Kudiezi Injection. Both groups were treated for 4 weeks. Before and after treat- ment,we detected blood sugar, blood fat and the change of blood theology, nerve conduction velocity, and calculated the clinical effective rates. Results: (1) Before and after treatment, we respectively tested patients' median nerve and phil total nerve conduction velocity. The results showed that compared with the control group, the experimental group had more obvious function of nerve conduction velocity and the results had statistical significance ( P 〈 0.05 ). (2) We detec- ted changes in lipid levels in both groups before and after treatment. Triglycerides (TG) and low - density lipoprotein cholesterol( LDL- C) were significantly decreased, and the high -density lipoprotein cholesterol (HDL -C) had the ob- vious rise. The changes in the experimental group were more obvious than those of the control group, and the difference was statistically significant (P 〈 0.05). And there were no obvious changes of serum total cholesterol(TC) (P 〉 0.05). (3) Before and after treatment,two groups' fasting blood glucose (FBG), 2 h postprandial blood glucose (2 HPG), gly- cosylated hemoglobin (HBA1 C) and so on sugar metabolism index changes had declined obviously. The results showed that the experimental group' s degree of decline was more obvious than that of the control group and the results had statis- tical significance(P 〈0.05). (4) After treatment, the test indexes of blood rheology in two groups decreased. But the experimental group' s decreased more significantly, and the difference was statistically significant ( P 〈 O. 05 ). (5) After treatment, according to the clinical total effective monitoring, the results of the experimental group( 82.85% ) was higher than the control group's (68.57%). Two groups symptoms improved significantly and the difference was statistically sig- nificant (P 〈 0. 05). Conclusion : Kudiezi Injection combined mecobalamine can improve blood sugar and blood lipids, the nerve function and the peripheral nerve excitability of patients with diabetic peripheral neuropathy. The treatment has a significant effect on diabetic peripheral neuropathy, worthy of clinical promotion.
