摘要
目的总结慢性酒精中毒性脑病的磁共振成像(MRI)特点及临床表现,提高对该病的认识。方法回顾性分析2015年8月至2017年4月解放军第二五一医院经临床证实的27例慢性酒精中毒性脑病患者的MRI检查结果及临床表现。对27例患者的MRI特征,包括常规MRI扫描及利用扩散张量成像进行归纳总结。结果 27例患者均有长期大量饮酒的嗜好,临床表现轻重不一。MRI可见患者出现不同程度的脑萎缩,其中6例合并韦尼克脑病,2例脑桥中央髓核溶解症,胼胝体形态信号发生变化。结论慢性酒精中毒性脑病患者因其在大脑受累位置不同,在临床上表现有明显差异;MRI对于该病具有明确的诊断价值。
Objective To summarize the characteristics of magnetic resonance imaging（ MRI） of chronic alcoholic encephalopathy and to improve the understanding of the disease. Methods The results of MRI examination and clinical manifestations of 27 patients with chronic alcoholic encephalopathy comfirmed by PLA 251 st Hospital from Aug. 2015 to Apr. 2017 were analyzed retrospectively. MRI features of the 27 patients including conventional MRI scans and diffusion tensor imaging were summarized. Results All of the 27 patients had long indulgence of drinking,with clinical manifestations of varying severities. MRI showed the patients had varying degrees of brain atrophy,of which 6 cases were combined with Wernicke encephalopathy,2 cases of central nucleus pulposus,with corpus callosum morphological changes. Conclusion Patients with chronic alcoholic encephalopathy have different clinical manifestations due to different affected locations in the brain; MRI has definite diagnostic value for chronic alcoholic encephalopathy.
