摘要 目的对Pi CCO血流动力学监测在重症肺炎合并心力衰竭患者中的应用进行研究。方法方便选取该院2016年8月—2017年12月收治的12例重症肺炎合并心力衰竭患者,对其入院救治过程中Pi CCO血流动力学监测以及液体复苏期间各项监测指标变化结果、患者疾病转归等情况进行统计分析。结果 Pi CCO血流动力学监测期间,患者MAP、CI、GEDI指标水平在实施液体复苏48 h后出现上升变化,分别为(84.9±11.7)mm Hg、(3.7±0.9)L/(min·m2)、(876.5±44.8)m L/m2,较液体复苏前差异有统计学意义(t=6.724、P<0.05,t=2.136、P<0.05,t=6.178、P<0.05);此外,经抢救治疗后,12例患者中,3例死亡,9例好转进入普通病房继续治疗,且生存与发生死亡患者的多项监测指标在液体复苏48 h差异有统计学意义(P<0.05)。结论对重症肺炎合并心力衰竭患者实施Pi CCO血流动力学监测,能够为患者液体复苏救治提供有效指导,避免各种不利影响发生,值得临床重视。 Objective The use of PiCCO hemodynamic monitoring in patients with severe pneumonia complicated with heart failure was studied. Methods 12 patients with severe pneumonia complicated with heart failure in the hospital from August2016 to December 2017 were selected to conduct statistical analysis on the hemodynamic monitoring of PiCCO during the course of hospital admission, and the results of changes in monitoring indicators and outcomes of patients during fluid resuscitation were concluded and analyzed. Results During the hemodynamic monitoring of PiCCO, the MAP, CI, and GEDI levels of patients showed an upward change after 48 h of liquid resuscitation(84.9±11.7)mm Hg,(3.7±0.9)L/(min·m^2),(876.5±44.8)m L/m^2, respectively, compared with the liquid before resuscitation, the difference was statistically significant(t=6.724,P〈0.05, t=2.136, P〈0.05, t=6.178, P〈0.05); In addition, the salvage treatment after that, of the 12 patients, 3 died and 9 improved to enter the general ward to continue treatment. The multiple monitoring indicators of survival and death patients showed significant differences at 48 hours after fluid resuscitation, with statistical significance(P〈0.05). Conclusion The PiCCO hemodynamic monitoring of patients with severe pneumonia combined with heart failure can provide effective guidance for the patient's liquid resuscitation and treatment, avoiding all kinds of adverse effects, and it is worthy of clinical attention.
