幽门螺杆菌对抗生素的耐药机制进展研究 被引量:9

摘要 伴随幽门螺旋杆菌(Hp)根除治疗法的逐步推广与使用,医学界开始关注抗生素耐药性越发严重的情况,该耐药性已被发现对一些Hp治疗方法的根治率有不利影响。因此,对幽门螺旋菌耐药分子机制的钻研正成为国内外研究的热点话题。例如,硝基咪唑类(甲硝唑)的耐药机制和rdx A、Frx A基因突变有重要关系。大环内酯类(克拉霉素)的耐药机制和Hp中23Sr RNA里V区域的点突变存在密切关系。阿莫西林一类药物的耐药机制和青霉素结合蛋白存在关联。作为快速抑菌剂的四环素类的耐药机制和Hp16Sr RNA序列的突变有关联。对耐药机制的分类研究为Hp的进一步诊断及医治奠定了重要基础。 With the gradual promotion and use of Helicobacter pylori(Hp) eradication therapy, the medical community has begun to pay attention to the increasingly serious situation of antibiotic resistance, which has been found to have an adverse effect on the cure rate of some Hp treatment methods. Therefore, the study of the molecular mechanism of Helicobacter pylori resistance has become a hot topic at home and abroad. For example, the resistance mechanism of nitroimidazoles(metronidazole) has an important relationship with mutations in rdx A and Frx A genes. The resistance mechanism of macrolides(clarithromycin) is closely related to point mutations in the V region of 23 S r RNA in Hp. The drug resistance mechanism of amoxicillin and penicillin-binding proteins is related. The resistance mechanism of tetracyclines as a rapid bacteriostatic agent is related to the mutation of the Hp16 Sr RNA sequence. The classification of drug resistance mechanisms has laid an important foundation for the further diagnosis and treatment of Hp.
