摘要
目的分析一氧化碳(CO)急性中毒后出现迟发性脑病(DEACMP)的危险因素。方法回顾性分析50例急性CO中毒患者的临床资料,根据临床记录和随访结果诊断DEACMP。收集的临床变量包括年龄、性别、CO暴露持续时间、意识丧失持续时间和实验室检查结果。比较DEACMP和非DEACMP2组间的变量。结果10例患者(20.0%)被诊断为DEACMP。DEACMP最常见的临床表现为认知功能减退,其次为缄默、帕金森综合征、尿失禁、癫痫和去皮层状态。DEACMP组的CO暴露时间显著大于非DEACMP组,2.5(0.5~8,25%~75%Q)h vs 0.3(0.3~0.5,25%~75%Q)h(P=0.007);同时DEACMP组的昏迷时间显著大于非DEACMP组,7.0(1.5~15.4,25%~75%Q)h vs 1.0(0.3~2.0,25%~75%Q)h,(P=0.004)。结论DEACMP是急性CO中毒患者常见的并发症;CO暴露时间和中毒后昏迷时间是DEACMP的发生的危险因素,同时对CO中毒患者进行神经心理症状的随访有利于其早期诊断。
Objective To analyze the risk factors of delayed encephalopathy (DEACMP) after CO poisoning after acute carbon monoxide (CO) poisoning. Methods The clinical data of 50 patients with acute CO poisoning were retrospectively analyzed,and DEACMP was diagnosed based on clinical records and follow up results.Clinical variables collected included age,gender,duration of CO exposure,duration of loss of consciousness,and laboratory findings.Compare variables between DEACMP and non DEACMP groups. Results Ten patients (20.0%) were diagnosed with DEACMP.The most common clinical manifestations of DEACMP are cognitive decline,followed by silence,Parkinson's syndrome,urinary incontinence,epilepsy,and de cortical state.The CO exposure time in the DEACMP group was significantly greater than that in the non DEACMP group,2.5(0.5 to 8,25% to 75% Q) h vs 0.3(0.3 to 0.5,25% to 75% Q) h( P =0.007);The coma time was significantly greater than the non DEACMP group,7.0(1.5 to 15.4,25% to 75% Q) h vs 1.0(0.3 to 2.0,25% to 75% Q)h,( P =0.004). Conclusion DEACMP is a common complication of patients with acute CO poisoning;CO exposure time and coma time after poisoning are risk factors for DEACMP,and the follow up of neuropsychological symptoms in CO poisoning patients is helpful for early diagnosis.
