外生型及内生型剖宫产切口妊娠MRI特征及诊断价值分析 被引量:2

摘要 目的探讨对外生型及内生型剖宫产切口妊娠患者实施磁共振成像(MRI)检查的效果,为临床诊断剖宫产切口妊娠提供指导。方法以2015年1月至2017年12月96例疑似剖宫产切口妊娠患者作为观察对象,进行回顾性分析。对96例疑似剖宫产切口妊娠患者均实施MRI检查、阴道超声检查,并以手术病理检查结果为参照依据,研究对比MRI检查的诊断价值。结果 MRI诊断剖宫产切口妊娠的敏感度、特异度、准确率分别为95.12%、92.86%、94.79%,阴道超声诊断剖宫产切口妊娠的敏感度、特异度、准确率分别为91.46%、85.71%、90.63%,组间比较无显著差异(P>0.05)。MRI检查诊断外生型及内生型剖宫产切口妊娠的符合率(95.12%)相比阴道超声(78.05%)明显更高(P<0.05);外生型剖宫产切口妊娠的瘢痕厚度、孕囊大小低于内生型剖宫产切口妊娠,血供丰富、包块型孕囊例数高于内生型剖宫产切口妊娠,差异有显著性(P<0.05)。结论外生型剖宫产切口妊娠患者的瘢痕厚度、孕囊体积相比内生型更小,孕囊与瘢痕接触面的长度相比内生型更大,血供相比内生型更丰富。对外生型及内生型剖宫产切口妊娠患者实施MRI检查切实可行,诊断效果较佳。 Objective To explore the effect of MRI on exogenous and endogenous cesarean section incision pregnancies to provide guidance for the clinical diagnosis of cesarean section incision pregnancy. Methods From January 2015 to December 2017,96 patients with suspected cesarean section incision pregnancy in our hospital were randomly selected as observation objects,and the clinical data of 96 suspected patients were retrospectively analyzed. MRI and vaginal ultrasonography were performed on 96 patients with suspected cesarean section incision. The results of surgical pathology were used as reference to study the diagnostic value of MRI. Results The sensitivity,specificity and total accuracy of MRI in the diagnosis of cesarean section incision were 95. 12%,92. 86% and 94. 79%,respectively. The sensitivity,specificity and total accuracy of vaginal ultrasound in the diagnosis of cesarean section pregnancy were 91. 46%,85. 71%,90. 63%,but there was no significant difference between the groups( P〈0. 05). The coincidence rate( 95. 12%) of MRI examination for diagnosis of exogenous and endogenous cesarean section incision pregnancy was significantly higher than that of vaginal ultrasound( 78. 05%)( P〈0. 05). The scar thickness,gestational sac size in exogenous cesarean section incision was lower than endogenous cesarean section incision pregnancy,blood supply was richer,mass of gestational sac was higher than endogenous cesarean section incision pregnancy( P〈0. 05). Conclusion Exogenous cesarean section incision patients with scar thickness,gestational sac volume is smaller than endogenous,the length of the gestational sac and scar contact surface is greater than endogenous,blood supply is more abundant than endogenous. The MRI examination of exogenous and endogenous cesarean section incision pregnancy is feasible and effective.
