摘要
目的:探讨与研究品管圈活动对临床恶性肿瘤进食困难患者营养状况和对吞咽功能的影响及应用价值。方法:选择四川省广元市中心医院2015年3月—2018年3月期间收治的恶性肿瘤进食困难者作为研究对象,其中2015年3月—2016年12月收治的研究对象为对照组(此期间我院未实施品管圈活动),共49人;2016年12月—2018年3月期间收治的研究对象为观察组(实施品管圈活动)。分析比较两组患者的吞咽情况、一般营养状况和护理满意度。结果:两组经干预后吞咽能力均有所上升,优于干预前,且干预后观察组吞咽功能显著优于对照组,差异显著(P<0.05);不同干预后,观察组患者的总蛋白(TP)、前白蛋白(PA)、白蛋白(ALB)、血红蛋白(Hb)水平显著优于对照组(P<0.05);两组的球蛋白水平无明显差异(P>0.05)。两组干预后较干预前自评项目、功能尺度显著提升,单项测量项目和症状尺度显著降低,且观察组改善效果明显优于对照组(P<0.05)。观察组患者护理服务满意度显著优于对照组,差异显著(P<0.05)。结论:对恶性肿瘤合并进食困难患者实施品管圈活动,可显著改善患者的吞咽状况和营养水平,提升患者生活质量,值得临床推广。
[Objective] To explore and study the effect and its application value of quality control circle activity on the general nutrition-al status and swallowing function of patients with dysphagia of malignant tumor. [Method] We chose 49 cases dysphagia of malignant tumor patients who take care in our hospital in March 2015 December 2016 (the period our hospital didn't carry out quality control circle activi-ties) as the control group ; the 49 cases dysphagia d malignant tumor patients who take care in our hospital in December 2016 March 2018 (the period our hospital carry out quality control circle activities) as the observation group. Analysis and comparison of swallowing, general nutritional status and nursing satisfaction between the two groups. [Result] After intervention, the swallowing ability d the two groups was increased, which was better than that before intervention, and the swallowing function of the observation group was significantly better than that of the control group after intervention ( P 〈 0. 05 ) . The levels of TP, PA, ALB and Hb in the observation group were significantly higher than those in the control group ( P 〈 0. 05), but there was no significant difference between the two groups in globulin levels ( P 〉 0. 05) . After intervention, the self-assessment items and functional scale of the two groups was higher than that before intervention and the single measurement items and symptom scales were significantly decreased, and the improvement effect of the observation group was signifi-cantly better than that of the control group (P 〈 0.05 ) . The satisfaction of nursing service in the observation group was significantly better than that of the control group ( P 〈 0.05 ) . [Conclusion] The implementation of quality control circle activities in patients with malignant tumors complicated with eating difficulties can improve the swallowing status and nutritional level of patients, improve the patients'life quali-ty, which is worthy of clinical promotion.
