摘要
目的探究微小RNA(miR)-455-5p和重组人半乳糖凝集素9(Galectin-9)在胆囊癌中的表达及临床意义。方法选取2010年4月至2012年10月收治的68例胆囊癌患者作为胆囊癌组,另选取胆囊良性病变60例作为良性病变组。实时荧光定量聚合酶链反应(qRT-PCR)法测定胆囊组织中Galectin-9、miR-455-5p、mRNA的表达,Western blot法、免疫组化法检测胆囊组织中Galectin-9蛋白表达水平及阳性表达率。分析miR-455-5p、Galectin-9表达与临床病理参数的关系。胆囊癌患者均进行5年随访,统计5年生存率,对影响患者生存率的临床因素进行COX回归分析,采用Kaplan-Meier生存曲线对生存情况进行分析,Log-Rank法检测组间生存差异。Pearson相关分析miR-455-5p、Galectin-9二者相关性。结果与胆囊良性病变组相比,胆囊癌组miR-455-5 mRNA表达显著升高,Galectin-9 mRNA、蛋白表达水平及蛋白阳性表达率显著降低(P<0.01)。胆囊癌组织中miR-455-5p、Galectin-9表达,与肿瘤分化程度、淋巴结转移有关(P<0.05,P<0.01)。COX单因素回归分析显示年龄、TNM分期、肿瘤分化程度、淋巴结转移、miR-455-5p表达、Galectin-9表达可能为影响患者预后的因素(P<0.05,P<0.01);多因素分析显示,TNM分期、肿瘤分化程度、淋巴结转移、miR-455-5p表达、Galectin-9表达为影响患者预后的独立因素(P<0.05,P<0.01)。miR-455-5p高表达者、Galectin-9阴性表达者平均生存期分别低于miR-455-5p低表达者、Galectin-9阳性表达者(P<0.01)。Pearson相关性分析显示,miR-455-5p与Galectin-9呈显著负相关(r=-0.573,P=0.000)。结论 miR-455-5p、Galectin-9与胆囊癌的发展有关,miR-455-5p高表达、Galectin-9低表达提示胆囊癌患者预后不良。
Objective To investigate the expressions of microRNA(miR)-455-5 p and recombinant human galactose agglutinin 9(Galectin-9) in gallbladder carcinoma and the clinical significance. Methods Sixty-eight patients with gallbladder carcinoma from April 2010 to October 2012 were selected as gallbladder carcinoma group,and 60 patients with benign gallbladder lesions were selected as benign gallbladder group. Quantitative real-time polymerase chain reaction(qRT-PCR) was used to detect the expressions of miR-455-5 p and Galectin-9 mRNAs in the gallbladder tissues. Western blot and immunohistochemical method were used to respectively detect the expression level of Galectin-9 protein and its positive expression rate in gallbladder carcinoma tissues. The relationship between the expressions of miR-455-5 p,Galectin-9 and the clinicopathological parameters were analyzed. Gallbladder carcinoma patients were followed-up,and 5-year survival rate was analyzed. COX regression analysis for the clinical factors of influencing survival rate was made. The kaplan-Meier survival curve was used to analyze survival status. Log-Rank method was used to detect the survival differences between groups. Pearson correlation analysis was used to analyze the correlation of miR-455-5 p and Galectin-9.Results The expression level of miR-455-5 mRNA in gallbladder carcinoma group was significantly higher than that in benign gallbladder lesion group,and the expression levels of Galectin-9 mRNA in the gallbladder carcinoma group was significantly lower than that in benign gallbladder lesion group(all P〈0. 01). The expression level and positive expression rate of Galectin-9 protein in gallbladder carcinoma group were significantly lower than those in benign gallbladder lesion group(all P〈0. 01). The expressions of miR-455-5 p and Galectin-9 in gallbladder carcinoma tissues were significantly associated with tumor differentiation degree and lymph node metastasis(P〈0. 05,P〈0. 01). COX single factor regression analysis showed that age,TNM staging,tumor differentiation degree,lymph node metastasis,miR-455-5 expression and Galectin-9 expression may be the factors affecting the prognosis of the patients(P〈0.05,P〈0. 01). COX multiple factor regression analysis showed that TNM staging,tumor differentiation degree,lymph node metastasis,miR-455-5 expression and Galectin-9 expression were the independent factors affecting patients' prognosis(P〈0.05,P〈0.01). The mean survival times in the patients with miR-455-5 p high expression and the patients with Galectin-9 negative expression were significantly lower than those in the patients with miR-455-5 p low expression and the patients with Galectin-9 positive expression,and there were significant differences in them for the comparison of survival curves(P〈0. 01). Pearson correlation analysis showed that miR-455-5 p was significantly negatively correlated with Galectin-9(r =-0. 573,P =0.000). Conclusion miR-455-5 p and Galectin-9 are related to the development of gallbladder cancer,and miR-455-5 p high expression and Galectin-9 low expression suggest the poor prognosis of gallbladder cancer patients.
