氧化型低密度脂蛋白、白三烯B4与糖尿病并发动脉粥样硬化患者血脂水平的相关性分析 被引量:7

摘要 目的探讨氧化型低密度脂蛋白(oxidized low density lipoprotein,oxLDL)、白三烯B4(leukotriene B4,LTB4)与糖尿病并发动脉粥样硬化(atherosclemsis,AS)患者血脂水平的相关性。方法选取2016年6月至2018年1月期间,我院内分泌科收治的125例2型糖尿病(type 2 diabetes mellitus,T2DM)患者作为研究对象。按照是否合并动脉粥样硬化分为T2DM+AS组和T2DM组,选取同期进行体检的健康人50例作为对照组。比较三组对象基本资料、血脂、血糖、oxLDL、LTB4水平,及其相关性分析。结果 T2DM组与T2DM+AS组的病程、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、空腹血糖(fasting blood glucose,FPG)、oxLDL、LTB4均显著高于对照组,高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)显著低于对照组(P<0.05);T2DM+AS组病程、TC、TG、LDL-C、FPG、oxLDL、LTB4显著高于T2DM组(P<0.05)。重度狭窄组和中度狭窄组的oxLDL、LTB4水平显著高于轻度狭窄组(P<0.05);重度狭窄组oxLDL、LTB4水平显著高于中度狭窄组(P<0.05)。经过Spearson相关性分析,oxLDL、LTB4与TC、TG、LDL-C、FPG呈正相关,与HDL-C呈负相关。结论 oxLDL、LTB4与糖尿病患者并发动脉粥样硬化患者血脂水平有密切关系,可明显促进动脉硬化进程。 Objective To investigate the correlation between levels of oxidized low-density lipoprotein (oxLDL), leukotriene B4 (leukotriene B4, LTB4 )and serum lipids in patients with diabetes mellitus complicated with atherosclerosis. Methods From June 2016 to January 2018, a total of 125 patients with type 2 diabetes mellitus (T2DM) admitted to our department of endocrinology were selected as study subjects. According to whether there was atherosclerosis, the patient was divided into the T2DM + AS group and the T2DM group, and 50 healthy people who underwent physical examination were selected as the control group at the same time. The basic data, blood lipids, blood glucose, oxLDL, and LTB4 levels in three groups of subjects were compared and their correlation analysis was performed. Results The course of disease, total cholesterol ( TC ), triglyceride ( TG ), low- density lipoprotein cholesterol ( LDL- C ), fasting blood glucose (FPG) ,oxLDL and LTB4 in the T2DM group and the T2DM + AS group were all significantly higher than that in the control group. High-density lipoprotein cholesterol(HDL-C) in the T2DM group and the T2DM + AS group was significantly lower than that in the control group ( P 〈 0.05 ). The course of disease, total cholesterol ( TC ), triglyceride ( TG ), low- density lipoprotein cholesterol ( LDL- C ), fasting blood glucose (FPG), oxLDL and LTB4 in the T2DM group were all significantly higher than that in the T2DM + AS group(P 〈 0.05 ). The levels of oxLDL and LTB4 in the severe stenosis group and the moderate stenosisgroup were significantly higher than those in the mild stenosis group ( P 〈 0.05 ). The oxLDL and LTB4 levels in the severe stenosis group were significantly higher than those in the moderate stenosis group ( P 〈 0.05 ). With Pearson correlation analysis, oxLDL and LTB4 were positively correlated with TC, TG, LDL- C, FPG and negatively correlated with HDL-C. Conclusion oxLDL and LTB4 are closely related to serum lipids levels in patients with diabetes mellitus complicated with atherosclerosis and can significantly promote the progression of atherosclerosis.
