超声心动图指标与病毒性心肌炎免疫功能相关性的研究 被引量:1

Research for correlation among echocardiography indexes and immune function in patients with viral myocarditis

摘要 目的:研究超声心动图指标LVEF,Tei指数与病毒性心肌炎(VMC)免疫功能的相关性。方法:选择2011年1月至2017年1月于本院诊治的VMC患者55例为VMC组,另选择同期55例健康体检者作为健康对照组。测量比较两组左室射血分数(LVEF)、Tei指数、心肌酶标志物、炎症因子以及免疫细胞水平。分析LVEF、Tei指数与炎症因子、免疫细胞水平的相关性。结果:与健康对照组比较,VMC组LVEF显著降低,Tei指数、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、天冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、高敏C反应蛋白(hsCRP)、肿瘤坏死因子(TNF)-α、干扰素(IFN)-γ、白介素(IL)-6、IL-8、白细胞计数(WBC)、淋巴细胞、CD3+、CD4+和CD8+T淋巴细胞水平均显著升高,P均=0.001。Spearman相关分析显示,LVEF与hsCRP、TNF-α、IFN-γ、IL-6、IL-8、WBC、淋巴细胞、CD3+、CD4+和CD8+T淋巴细胞水平呈显著负相关(r=-0.402~-0.119,P<0.05或<0.01),Tei指数与上述指标水平呈显著正相关(r=0.127~0.433,P<0.05或<0.01)。结论:病毒性心肌炎患者存在免疫功能紊乱,超声心动图测定的患者LVEF,Tei指数与其免疫功能有一定相关性,值得重视。 Objective: To study correlation among echocardiography indexes and immune function in patients with viral my- ocarditis (VMC). Methods: A total of 55 VMC patients, who were treated in our hospital from Jan 2011 to Jan 2017, were selected as VMC group. Another 55 healthy subjects undergoing physical examination in our hospital during the same period were selected as healthy control group. Left ventricular ejection fraction (LVEF), Tel index, myocardial enzymes, inflam- matory factors and immune cell levels were measured and compared between two groups. The correlation among LVEF, Tei index, inflammatory factors and immune cell levels were analyzed. Results: Compared with healthy control group, there was significant reduction in LVEF, and significant rise in Tei index, creatine kinase (CK), creatine kinase isoenzyme MB (CK-MB), aspartate transaminase (AST), lactate dehydrogenase (LDH), high sensitive C reactive protein (hsCRP), tumor necrosis factor (TNF) -α, interferon (IFN) -7, interleukin (IL) -6, IL-8, white blood cell counting (WBC), lym- phocytes, CD3 + , CD4 + and CD8 + T lymphocyte levels in VMC group, P = 0. 001 all. Spearman correlation analysis in- dicated that LVEF was significant inversely correlated with levels of hsCRP, TNF-α, IFN-7, IL-6, IL-8, WBC, lympho- cytes, CD3 +, CD4 + and CD8 + T lymphocytes ( r = - 0. 402~ - 0. 119, P(0.05 or 〈0.01), and Tei index was signifi- cant positively correlated with above indexes (r = 0. 127~0. 433, P〈0. 05 or 〈0. 01). Conclusion: Immune dysfunction exists in patients with viral myocarditis. There exist correlation among echocardiography measured LVEF, Tei index and immune function in patients with viral myocarditis, which is worth paying attention.
