摘要
目的 观察左甲状腺素钠片对妊娠伴甲状腺功能减退患者脂代谢及妊娠期并发症的影响.方法 选取2016年3月至2017年8月140例妊娠伴甲状腺功能减退患者进行前瞻性研究,按随机数表法分为观察组及对照组,每组70例.对照组予以基础常规药物治疗,观察组在对照组基础上予以左甲状腺素钠片治疗.比较两组治疗前后的游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)和促甲状腺激素(TSH)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、妊娠并发症总发生率与不良妊娠结局总发生率.结果 观察组治疗后的FT3与FT4均高于治疗前及对照组(P〈0.05),TSH低于治疗前及对照组(P〈0.05).观察组治疗后的TC、TG、LDL-C均低于治疗前及对照组(P〈0.05),HDL-C高于治疗前及对照组(P〈0.05).观察组妊娠并发症总发生率(2.86%)显著低于对照组(11.43%),差异有统计学意义(P〈0.05);两组不良妊娠结局总发生率分别为1.43%和7.14%,差异未见统计学意义(P〉0.05).结论 左甲状腺素钠片治疗妊娠伴甲状腺功能减退患者可有效改善其脂代谢及甲状腺功能,减少妊娠期并发症.
Objective To observe the effect of left thyroxine sodium tablets on lipid metabolism and pregnancy complications during pregnancy with hypothyroidism. Methods A total of 140 patients with pregnancy associated with hypothyroidism from March 2016 to August 2017 were selected for pro-spective study. The patients were divided into observation group and control group according to random number table method, with 70 cases in each group. The control group was given basic routine drug treat- ment, and the observation group was treated with levothyroxine sodium tablets on the basis of the control group. The levels of free triiodothyronine ( FT3 ) , free thyroxine ( FT4 ) and thyrotropin ( TSH ) , total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol LDL-C ), high-density lipopro- rein cholesterol ( HDL-C ) , the total incidence of pregnancy complications and the overall incidence of adverse pregnancy outcomes were compared between the two groups. Results The levels of FT3 and FT4 in the observation group were higher than those before treatment and the control group (P 〈 0.05 ) , while the levels of TSH in the observation group was lower than that before treatment and the control group (P 〈0. 05 ). The levels of TC, TG and LDL-C in the observation group were lower than those before treatment and the control group ( P 〈 0. 05 ), the levels of HDL-C was higher than that before treatment and the control group (P 〈 0. 05 ). The total incidence of pregnancy complications in the observation group (2.86%) was significantly lower than that in the control group ( 11.43% ), and the difference was significant ( P 〈 0.05 ). The total incidence of adverse pregnancy outcomes in the two groups was 1.43 % and 7. 14%, respectively, the difference was not significant (P 〉 0. 05 ). Conclusions Levothyroxine sodium tablets in patients with pregnancy associated with hypothyroidism can effectively improve their lipid metabolism and thyroid function, and reduce the complications during pregnancy.
