产气肠杆菌药敏情况分析及Ampc酶基因型检测

Analysis of antimicrobial susceptibility of Enterobacter aerogenes and detection of Ampc enzyme genotype

摘要 目的了解本院产气肠杆菌临床耐药情况,并做产AmpC酶基因型研究,进一步指导临床合理用药。方法总结本院132株产气肠杆菌科室分布及常用抗生素耐药情况,筛选出产AmpC酶菌株,PCR检测相关酶基因表达。结果菌株大多来源于神经外科、重症医学科及呼吸内科标本,分别占比38.64%、26.52%和16.67%;本院分离的产气肠杆菌对头孢唑啉和头孢哌酮耐药率分别高达84.09%和78.03%,其对头孢西丁耐药率为29.55%;对碳青霉烯类抗生素亚胺培南和美罗培南敏感率分别为93.18%和94.70%;共检测出32株表达AmpC酶基因,其中ACT-1型25株,DHA型5株,ADC型2株,未检出ACC型和MOX型。结论产气肠杆菌耐药现象较为严峻,本院产AmpC酶菌株大多为ACT-1基因型。 Objective To understand the clinical drug resistance of Enterobacter aerogenes in our hospital and to make the study of AmpC enzyme genotype to further guide the clinical rational drug use. Methods The distribution of 132 strains of Enterobacter aerogenes and the antibiotic resistance were summarized. The AmpC strain was screened and the expression of related enzymes was detected by PCR. Results Most of the strains were isolated from specimens in neurosurgery,ICU and respiratory,accounting for 38. 64%,26. 52% and 16. 67% respectively. The resistance rate to cefazolin and cefoperazone were 84. 09%and 78. 03% respectively. The antibiotic resistance rate was 29. 55% to cefoxitin. The sensitivity of carbapenems and amines were 93. 18% and 94. 70% respectively. AmpC genes were detected in 32 strains,including 25 strains of ACT-1,5 strains of DHA and 2 strains of ADC,and the ACC type and MOX type were not detected. Conclusion The antibiotic resistance of Enterobacter aerogenes is more severe. Most of the AmpC enzyme strains are ACT-1 genotype.
