摘要
目的探讨负压封闭引流(VSD)联合高压氧治疗(HBOT)对难愈性创面的影响。方法回顾性分析2017年1月—2017年12月收治150例难愈性创面患者的临床资料。根据不同治疗方式将患者分为A、B和C组,每组50例。A组采用传统换药方法,B组采用单独VSD治疗,C组采用VSD联合HBOT治疗。比较3组的临床疗效和治疗相关指标。结果 B组和C组的临床总有效率和血管内皮生长因子的标记指数明显高于A组,且C组高于B组(P<0.05)。B组和C组的伤口愈合时间、住院时间以及肉芽组织覆盖创面80%以上所用的覆盖时间明显短于A组,且C组短于B组(P<0.05)。结论采用VSD联合HBOT治疗难愈性创面的临床效果显著,能提高血管内皮生长因子的表达,加速创面的愈合。
Objective To investigate effect of vacuum sealing and drainage (VSD) combined with hyperbaric oxygenation (HBOT) on refractory wounds. Methods Clinical data of 150 patients with refractory wounds admitted during January 2017 and December 2017 was retrospectively analyzed, and the patients were divided into group A, B and C ( n =50 for each group) according to different treatment methods. Group A was treated with traditional dressing changes, and group B was treated with VSD alone, while group C was treated with VSD combined with HBOT. Clinical efficacy and treatment related indexes were compared among 3 groups. Results Values of the clinical total effective rate and labelling index of vascular endothelial growth factor in group B and C were significantly higher than those in group A, and the values in group C was significantly higher than those in group B ( P 〈0.05). Times of wound healing, hospitalization and duration for more than 80% of the granulation tissue covered wound in group B and C were significantly shorter than those in group A, and the times in group C were significantly shorter than those in group B ( P 〈0.05). Conclusion VSD combined with HBOT is effective in treatment of refractory wounds. It may improve expression of vascular endothelial growth factor and accelerate wound healing.
