摘要
目的了解评估2岁以内先天性肛门直肠畸形(anorectal malformations,ARM)患儿术后的生活质量,并分析其影响因素。方法随访2014年9月至2018年2月经我院治疗的2岁以内ARM术后患儿,采用中文版Peds QLTM家庭信息表格(Peds QLTMFamily Information Form)和Peds QLTM婴儿生活质量量表(Peds QLTMInfant Scale)对生活质量进行评价,比较ARM术后患儿与健康儿童生活质量的差异,使用单因素分析和多元逐步回归分析对ARM术后患儿生活质量影响因素进行分析。结果共回收问卷128份,纳入调查ARM术后患儿68例,健康儿60例。ARM组生活质量总分为(89.15±4.60)分,健康儿组生活质量总分为(86.08±13.52)分。其中两组情感功能维度得分比较,差异有统计学意义(P<0.05)。肛门直肠畸形类型是影响ARM患儿生理健康得分的主要因素。结论 2岁以内的ARM术后患儿生活质量总体与同龄健康儿相似,针对不同疾病类型的患儿治疗护理侧重点应区别对待,实施个体化的方案,从而促进患儿的康复,提高生活质量。
Objective To evaluate the postoperative health-related quality of life ( HRQoL) and analyze the influencing factors in infants aged from 1 to 24 months with congenital anorectal malformations ( ARM ).Methods Postoperative infants younger than 2 years old with ARM were followed up and estimated using the Family Information Form (PedsQL) and Infant Scale (PedsQL) between September 2014 and February 2018.Age-matched healthy infants were used as controls.The differences in quality of life were compared between the two groups.The influencing factors of HRQoL were analyzed by using single-factor analysis and multivariate re-gression analysis.Results One hundred and twenty-eight participants with 68 ARM and 60 controls were included.There were no significant differences in total scale score between the ARM group ( 89.15 ±4.60 ) and control group ( 86.08 ± 13.52 ).The emotional function score of ARM group was significantly higher than that in the control group ( P 〈0.05 ).The type of ARM was associated with the physical health in infants with ARM.Conclusion The HRQoL is comparative between postoperative ARM infants and healthy in-fants.Individualized nurse plan based on the type of ARM should be applied to improve the future HRQoL.
