摘要
目的探讨激光联合药物治疗在后发性白内障中的应用价值,并分析其对患者角膜内皮细胞的影响。方法选择86例(86眼)为研究对象,随机分为研究组及对照组,每组43例(43眼),对照组采用Nd:YAG激光治疗,研究组在对照组的基础上术后加用药物进行治疗,比较2组患者治疗前后角膜内皮细胞及泪液白细胞介素-4(IL-4)、白细胞介素-10(IL-10)指标变化,并观察2组患者视力改善情况。结果治疗前,2组术前视力≥0.3的人数比较差异无统计学意义(P>0.05);研究组患者术后1 d视力≥0.3、术后7 d视力≥0.3的人数均明显高于对照组(P<0.05)。治疗后7 d,2组细胞密度与六角形细胞比例均明显低于治疗前(P<0.05),变异系数则明显高于本组治疗前(P<0.05);但治疗后7 d 2组细胞密度、变异系数与六角形细胞比例比较差异无统计学意义(P>0.05)。治疗前,2组患者泪液IL-4与IL-10指标比较差异无统计学意义(P>0.05);治疗7 d后,研究组患者泪液IL-4与IL-10指标明显低于对照组(P<0.05)。结论 Nd:YAG激光联合药物治疗后发性白内障患者效果显著,可有效改善患者的视力水平,降低泪液IL-4与IL-10炎症指标,对角膜内皮细胞没有明显的影响,值得应用。
Objective To investigate the clinical application value of laser combined with drugs in treatment of cataract,and to observe its effects on corneal endothelial cells of patients. Methods A total of 86 patients with cataract( 86 eyes) were enrolled as research objects,who were randomly divided into research group( n = 43,43 eyes) and control group( n = 43,43 eyes). The patients in control group were treated by Nd: YAG laser treatment,however,the patients in research group,on the basis of the control group,were treated by oral drugs after operation. The changes of interleukin-4( IL-4),interleukin-10( IL-10) in corneal endothelial cells and tear before and after treatment,as well as visual acuity improvement status were observed and compared between the two grousp. Results Before treatment,there were no significant differences in the numbers of patients with visual acuity≥0. 3 between two groups( P〈0. 05). After treatment the numbers of patients with visual acuity≥0. 3 at 1 d after operation and those with visual acuity ≥ 0. 3 at 7 d after operation in research group were significantly more than those in control group( P〈0. 05). Moreover the cell density and percentage of hexagonal cells at 7 d after treatment were significantly lower than those before treatment( P〈0. 05),while the coefficient of variation was significantly higher than that before treatment in each group( P〈0. 05). There were no significant differences in cell density,coefficient of variation and percentage of hexagonal cells at 7 d after treatment between two groups( P〈0. 05). Before treatment,there were no significant differences in the levels of IL-4 and IL-10 between two groups( P〈0. 05),however,the levels of IL-4 and IL-10 at 7 d after treatment in research group were significantly lower than those in control group( P〈0. 05). Conclusion Nd: YAG laser combined with drugs in treatment of cataract can obtain obvious therapeutic effects,which can effectively improve the visual acuity of patients,reduce inflammation factors including IL-4 and IL-10,moreover,which has no obvious effects on corneal endothelial cells,therefor,it is worthy of clinical application.
