摘要
目的比较孤立性髂动脉瘤(IIAA)超声影像与CT血管造影(CTA)的表现,探讨超声检查在IIAA诊断中的应用价值。方法回顾性分析2014年11月至2017年11月在我科行超声检查的IIAA患者19例,患者均同时进行CTA检查,对动脉瘤的位置,形态、附壁血栓及最大直径等进行对比分析。结果超声检查发现髂总动脉瘤25个,其中伴后壁溃疡2个,伴附壁血栓11个,与CTA检查结果完全一致,其准确率、敏感性及特异性均达到100%;对于累及髂内动脉的IIAA,超声检查发现髂内动脉瘤为6个,CTA检查为7个,超声诊断髂内动脉瘤的准确率、敏感性为86%,特异性为100%;超声发现髂内动脉瘤伴附壁血栓4个,CTA检查为5个,超声诊断髂内动脉瘤附壁血栓的准确率、敏感性为80%,特异性为100%。对超声检查发现的25个髂总动脉瘤及6个髂内动脉瘤最大径及附壁血栓厚度与CTA测量数值对比分析差异无统计学意义(P>0.05)。结论应用超声对IIAA尤其对于诊断孤立性髂总动脉瘤进行术前诊断,具有较高的准确性、敏感性及特异性,超声对于IIAA患者的术前检查及筛查方面具有非常重要的应用价值。
Objective To compare the capability of ultrasound and computed tomography angiography （ETA） in evaluating the isolated lilac artery aneurysm （IIAA） and discuss the application value of ultrasound in the diagnosis of IIAA. Methods Retrospective analysis of the ultrasound applications for 19 cases with IIAA diagnosed by ETA in our department from November 2014 to November 2017. The location, form ,range of the artery aneurysm and the lateral thrombus were compared. Results There were 25 common lilac aortic aneurysms, accompanied by 2 posterior wall ulcer, 11 lateral thrombus, and ETA had the same results. The accuracy, sensitivity and specificity were 100%. For IIAA involving the internal lilac artery, the results of ultrasound showed that there were 6, 7 were diagnosed by ETA. The accuracy and sensitivity of ultrasound were 86%, specificity was 100%. The ultrasound showed that there were 4 lateral thrombus of the internal lilac artery, 5 were diagnosed by ETA. The accuracy and sensitivity of ultrasound were 80%, specificity was 100%. The largest diameter and the thickness of lateral thrombus for 25 common lilac aortic aneurysms and 6 internal lilac aortic aneurysms measured was no statistical difference between ultrasound and ETA （P〉0.05）. Conclusions The application of ultrasound for the diagnosis of IIAA has high accuracy, sensitivity and specificity, especially for the isolated common lilac artery aneurysm. The ultrasound is worthy of application in IIAA for the preoperative diagnosis and screening.
