丙泊酚联合右美托咪啶与芬太尼在无痛气管镜检查中的应用价值 被引量:2

摘要 目的探讨丙泊酚联合右美托咪啶与芬太尼在无痛气管镜检查中的应用价值。方法选取该院2016年7月—2017年7月进行无痛气管镜检查的患者100例,随机分为两组,对照组患者使用丙泊酚进行麻醉,实验组患者在使用丙泊酚麻醉的基础上,使用美托咪啶与芬太尼联合对患者进行处理,对两组患者血流动力学相关指标进行检测,对两组患者麻醉效果及麻醉后不良反应例数进行记录。结果实验组麻醉监测后心率(89.23±8.57)次/min、氧饱和度(92.31±0.31)%、收缩压(118.21±10.76)mm Hg、舒张压(63.43±6.23)mm Hg;对照组麻醉监测后心率(71.88±7.08)次/min、氧饱和度(90.59±0.36)%、收缩压(74.42±6.89)mm Hg、舒张压(60.32±5.56)mm Hg。以上数据实验组显著高于对照组,差异有统计学意义(t=5.39、3.89、4.82、6.49,P<0.05)。实验组麻醉效果起效时间(18.31±3.21)min、镇痛维持时间(33.21±5.18)min,对照组麻醉效果起效时间(40.53±5.68)min、镇痛维持时间(21.48±2.89)min,实验组麻醉效果起效时间显著低于对照组,差异有统计学意义(t=7.39,P<0.05);实验组镇痛维持时间显著高于对照组,差异有统计学意义(t=5.27,P<0.05)。实验组患者出现不良反应例数为4例(8.00%),对照组患者为12例(24.00%),实验组患者使用的麻醉方式更有效(χ~2=6.40,P>0.05)。结论临床对无痛气管镜检查的患者,可以使用丙泊酚联合右美托咪啶与芬太尼对患者进行处理,对患者检查应用效果更好。 Objective To study the application value of propofol combined with dexmedetomidine and fentanyl in the painless tracheoscopy.Methods 100 cases of patients with painless tracheoscopy in our hospital from July 2016 to July2017 were selected and randomly divided into two groups,the control group used the propofol for anesthesia,while the experimental group used the medetomidine and fentanyl for treatment on the basis of the control group,and the hemodynamics related indicators were tested,and the anesthesia effect and adverse reaction cases after anesthesia were recorded.Results The heart rate after anesthesia and monitoring,oxygen saturation,systolic pressure and diastolic pressure in the experimental group were obviously higher than those in the control group [(89.23 ±8.57)times/min,(92.31±0.31)%,(118.21±10.76)mm Hg,(63.43±6.23)mm Hg vs(71.88±7.08)times/min,(90.59±0.36)%,(74.42±6.89)mm Hg,(60.32±5.56)mm Hg],and the differences were statistically significant(t=5.39,3.89,4.82,6.49,P〈0.05),and the anesthesia onset time in the experimental group was obviously lower than that in the control group[(18.31±3.21)min vs(40.53±5.68)min],the difference was statistically significant(t=7.39,P〈0.05).and the analgesia duration in the experimental group was obviously higher than that in the control group,[(33.21±5.18)min vs(21.48 ±2.89)min],and the differences were statistically significant(t=5.27,P〈0.05),and the incidence rate of adverse reactions in the experimental group and in the control group was respectively 4 cases(8.00%) and 12 cases(24.00%),and the difference was statistically signifi cant(χ2=6.40,P〈0.05).Conclusion The propofol combined with dexmedetomidine and fentanyl in the painless tracheoscopy can have a better application effect of patients.
