摘要
目的:临诊收集绝经综合征患者中医四诊信息,并分析患者的中医证候分布特点。方法:根据本课题组前期的文献研究,建立绝经综合征中医临床诊断记录表,收集367例患者的中医症状、体征、舌象、脉象等四诊信息,并对其进行统计分析。结果:证候分布表现为肝肾阴虚证>肝郁气滞证>心肾不交证>心脾两虚证>脾肾阳虚证;症状分布表现为烘热汗出>腰膝酸软>弦脉>头晕>健忘>耳鸣>心悸怔仲>倦怠乏力等;舌脉象分布:肝肾阴虚证以舌质红、少苔为主,肝郁气滞证以舌淡红、苔薄黄为主,脾肾阳虚证以舌淡白、苔薄白为主,心脾两虚证以舌淡红、苔薄白为主,心肾不交证以舌淡红、苔薄白为主;各证型脉象以不同程度弦脉为主;绝经综合征各证型有其特征性的舌象、脉象定性及定量指标;且各证型之间舌象、脉象定量指标比较,差异有统计学意义(P<0.05)。结论:绝经综合征证型临证分布,肝肾阴虚证居第一位,其次为肝郁气滞证、心肾不交证,其症状、舌象、脉象也因证候分布规律而呈现不同的分布特征。
Objective： To collect the four diagnostics information of TCM in patients with menopausal syndrome, and analyze the distribution characteristics of TCM syndromes. Methods： According to our previous studies, the clinical diagnosis of TCM records were made. And 367 cases of patients with clinical symptoms, signs, tongue, pulse and other diagnostic information were collected, and made a statistic analysis for these parameters. Results： The TCM syndrome distribution： liver kidney yin deficiency syndromeliver qi stagnation syndromedisharmony of heart and kidney deficiency syndromeheart and spleen deficiency syndromespleen and kidney yang deficiency syndrome; The symptoms distribution： the detonation heart and sweatingsoreness and weakness of waist and kneestaut pulsedizzinessforgetfulnesstinnitusterrified palpitationfatigue and other symptoms; The tongue and pulse distribution： liver kidney yin deficiency syndrome with red tongue and less moss, liver qi stagnation syndrome with pale tongue, thin and yellow moss, spleen and kidney yang deficiency syndrome with pale tongue, thin white fur, heart and spleen deficiency syndrome with pale tongue, thin white fur, disharmony of heart and kidney syndrome with pale tongue, thin white fur; the pulse pattern of each syndrome was mainly based on different degrees of miry pulse; each type of menopause syndrome had its characteristic tongue condition, pulse qualitative and quantitative indexes; and there was a significant difference in tongue and pulse condition between each syndrome type（P〈0.05）. Conclusion： The distribution of clinical syndromes of menopausal syndrome, liver kidney yin deficiency syndrome ranks first, followed by liver qi stagnation syndrome, disharmony of heart and kidney syndrome. Its symptoms, tongue image and pulse condition also shows different distribution characteristics because of syndrome distribution.
